Andrea Fischer returns today for reevaluation regarding hearing loss and nasal obstruction. She has had no significant relief
despite medical therapy.
Her evaluation is significant for retracted tympanic membrane on the right side. She has a bilateral mixed hearing loss with
a conductive component on the right. In addition, she has nasal septal deviation, boggy turbinates, and swelling of the nasal
mucosa. A CAT scan showed inflammatory changes in the maxillary sinuses.
l recommended right myringotorny and tube with septoplasty, turbinectomy, and limited sinus surgery. We will try to perform
this at her conveniences l have asked her to obtain medical clearance you in preparation for general anesthesia especially in
regard to her history of congenital heart disease.
