4. Summary
Background: Recent developments in large bowel surgery are the introduction of laparoscopic
surgery and the implementation of fast track recovery multimodal programs. Both programs
focus on faster recovery and shorter hospital stay.
Objectives: To determine whether laparoscopic surgery, fast track perioperative care or a
combination of both is to be preferred over open surgery with standard care in patients having
segmental colectomy for malignant disease.
Patients and Methods: double blinded, multicenter trial with a 2 x 2 balanced factorial design.
Patients eligible for segmental colectomy for malignant colorectal disease viz. right and left
colectomy and anterior resection will be randomised to either open or laparoscopic colectomy,
and to either standard care or the fast track program. This factorial design produces four
treatment groups (a) open colectomy with standard care (b) open colectomy with fast track
program (c) laparoscopic colectomy with standard care and (d) laparoscopic surgery with fast
track program. Primary outcome measure are length of postoperative hospital stay including
readmission within 30 days, quality of life two weeks after surgery, overall hospital costs.
Secondary outcome parameters are morbidity, patient satisfaction and readmission.
Data analysis: We anticipate a difference of 4 days between standard care and the fast track
laparoscopic group. Based on a mean postoperative hospital stay of 9 +/- 2.5 days a group
size of 400 patients (100 each arm) can reliably detect a minimum difference of 1 day
between the four arms (alfa = 0.95, beta = 0.8). With 100 patients in each arm a difference of
10% in subscales of the SF-36 and social functioning can be detected.

5. Introduction
Yearly, 9500 segmental colonic resections are done for benign and malignant disease
(PRISMANT code 5455). Recent developments in large bowel surgery are the introduction of
laparoscopic surgery and the implementation of fast track recovery multimodal programs.
Both programs focus on faster recovery and shorter hospital stay. Laparoscopic colectomy
was first described in 1991. Since then a lot of effort has been done to establish its feasibility
and safety particularly in laparoscopic colectomy for cancer. Recently, several randomised
trials comparing laparoscopic with open colectomy indicated that laparoscopic surgery can be
safely applied both for malignant as well as benign indications 1-5. In a meta-analysis, less
morbidity, less postoperative pain, earlier recovery and shorter hospital stay were reported for
the laparoscopic approach of colorectal cancer 6. These results stimulated many surgeons in
the Netherlands to set up a laparoscopic colorectal program.
At the same time, enthousiasm was raised for the so-called fast track recovery multimodal
program developed by a danish surgeon Henrik Kehlet 7-11. This multimodal program
involving optimalisation of several aspects of the perioperative management of patients
having colectomy enables patients to go home as early as three days after open colectomy.
Preoperative counseling of the patient, tailored anesthesiology encompassing thoracic
epidural anesthesia, fluid restriction and non-opiod pain management, early postoperative
feeding and mobilisation enabled not only an early discharge after surgery, also morbidity
was reduced compared to standard perioperative care 12-17.
Both laparoscopic surgery and fast track programs are costly and require extensive expertise.
Laparoscopic surgery is costly due to expensive disposibles and additional operating time, and
a considerable learning curve must be mastered. Only 3% of the colectomies are therefore
done laparoscopically in the Netherlands. Fast track multimodal perioperative care requires
additional personnel trained in the several aspects of the fast track program, to make the
program work. Fast track multimodal programs are running in no more than five dutch
hospitals.
As these new developments have been introduce in clinical practice, time has come to
evaluate its cost-effectiveness in large bowel surgery. It is clear that both laparoscopic surgery
and fast track programs reduce hospital stay. The largest reduction in hospital stay can
probably be achieved by a combination of fast track programs and laparoscopic surgery. It is
not known what the additional costs of laparoscopic surgery or fast track programs are

6. compared to the reduction in hospital stay that can be achieved with these programs. Since the
average postoperative hospital stay after segmental colectomy is still 17 days in the
Netherlands, an enormous improvement can be expected applying fast track programs and/or
laparoscopy. What the relative contribution is in reduction in hospital stay of both methods is
unknown. This must be assessed in a setting where patients are blinded for the approach of
surgery. Implementation of fast track programs is relatively simple since it only requires
additional effort of the medical staff. Implementation of laparoscopic colectomy will take
more time, since an extensive learning curve must be mastered.
It can be hypothesized that fast track and/or laparoscopy are associated with less attenuation
of the patient s condition after surgery resulting in a more rapid postoperative stay and faster
recovery to full activity at home. This will be assessed using validated quality of life
questionnaires.
Since it has not been established which combination of perioperative management viz.
standard care, fast track colonic surgery, laparoscopic colonic surgery or laparoscopy
combined with fast track is best in terms of postoperative stay, quality of life, postoperative
morbidity, overall costs and patient satisfaction, it is the subject of the present study proposal.
Research questions and objectives:
To determine whether laparoscopic surgery, fast track perioperative care or a combination of
both is to be preferred over open surgery with standard care in patients having segmental
colectomy for malignant disease.
Research questions:
1. How does laparoscopic surgery compare with open surgery in terms of hospital stay,
quality of life and costs?
2. How does fast track perioperative care compare with standard care in terms of hospital stay,
quality of life, and costs?
3. What is the added benefit of fast track perioperative care in laparoscopic surgery in terms
of hospital stay, quality of life, and costs?

7. Patients and Methods
Study design:
We propose a 2x2 balanced factorial design to evaluate whether laparoscopic surgery, fast
track perioperative care, or a combination of both is better than open surgery with standard
care in patients having segmental colectomy for malignant disease in multicenter setting.
Patients are blinded for the type of intervention viz. laparoscopic or open.
Study population:
Patients eligible for segmental colectomy for malignant colorectal disease viz. right and left
colectomy and anterior resection
Inclusion criteria
- age between 40 and 80 years
- colorectal cancer including colon and rectosigmoid cancers
- informed consent
Exclusion criteria:
- prior midline laparotomy
- ASA IV
- laparoscopic surgeon not available
- prior upper and/or lower midline laparotomy
- emergency colectomy
- planned stoma
Study outline:
Informed consent will be obtained at the outpatient department if the patient fullfills the in
and exclusion criteria. Randomisation can be done instantly through the study website.
This factorial design produces four treatment groups (a) open colectomy with standard care
(b) open colectomy with fast track program (c) laparoscopic colectomy with standard care and
(d) laparoscopic surgery with fast track program.
Patients that are included in the fast track protocol (appendix A) will be informed by a fast
track trial nurse and the anesthesiologist about the fast track program. Appointments for
these consultations will be made after consulting the surgeon and randomisation has been
done. All patients randomised to have a fast track perioperative treatment will be admitted at a

8. separate fast track ward, where the nurses and medical staff is trained in fast track
perioperative management. Fast track and standard care patients will be taken care of in
separate quot;fast trackquot; and quot;standard carequot; wards.
Patients who will receive standard treatment are not counseled by the fast track nurse and will
have a standard preassessment by the anesthesiologist.

Patient and medical staff will be blinded for the surgical approach until the day of discharge
by applying a covering abdominal bandage.
Surgery:
Both open and laparoscopic surgery is done according to the technique applied by the local
surgeon. Antibiotic prophylaxis is done according the hospital protocol. Patients having
laparoscopic surgery or fast track surgery will have two enemas before surgery (evening
before, and morning before). The open standard care group will have bowel preparation
according the hospital protocol. After surgery the surgical wounds are covered with a
abdominal dressing in order to blind the medical staf for the type of approach.
A requirement to perform laparoscopic colectomy for cancer is a minimum of 20
laparoscopic colectomies for benign disease as indicated by the proclamation of the American
Society of the Colon and Rectum Surgeons in 2004. M.A. Cuesta and W.A. Bemelman will
train the participating surgeons throughout 2005 to cope with these minimum criteria
whenever necessary in the participating center. To indicate the institutional expertise in
laparoscopic colectomy in the participating centers, the number of laparoscopic colectomies is
listed in Appendix B.
Fast track and standard care:
Comparison of the different strategies is only possible when a fast track program is running
sufficiently and patients are nursed separately depending on the results of randomisation
either on a standard care or fast track ward in order to avoid a bias towards fast track
treatment by the nursing and medical staff. Patients that have standard care cannot be nursed
by nurses that have experience with fast track care. This has been a problem in some hospitals
where fast track has already been implemented. In the AMC there is one ward that is
accustomed to fast track, another that is not. In the hospitals where a fast track program has to
be institutionalised, it is still possible to nurse fast track patients separately from the standard

9. care patients. A team from the AMC consisting of a surgeon, anaesthesist and nurse will help
to implement and organise the fast track program in the other hospitals and will coach the
implementation throughout 2005. After this implementation, when the protocol is properly
running and there is sufficient protocol compliance the randomisation and inclusion of the
first patient starts. Fast track multimodal management is done according the attached protocol
(Appendix A). This protocol is running in the AMC after a pilot study was held in the AMC.
Data collection:
Appendix C describes the data collection including patient characteristics, OPOSSUM score,
surgical parameters, morbidity, lenght of stay, blood samples, quality of life, etc,
Discharge criteria:
Since hospital stay is a primary efficacy parameter, the discharge criteria must be defined.
Every postoperative day will be noted whether the discharge criteria are met, and other
reasons of prolonged hospital stay viz. social environnement or patient inacceptance.
Criteria:
- adequate pain control with oral analgetics
- no nausea
- ability to take solid foods
- passage of flatus and/or stool
- mobilisation and self support as preoperative
- acceptance from patient

Power/data-analysis:
Since both fast track and laparoscopy focus on reduction of hospital stay, this is used as
primary efficacy parameter.The mean postoperative hospital stay for segmental bowel
resection with standard care is 9 days with a s.d. of 2.5 in the AMC. Using a 5% significance
level, a total sample size of 400 would have a power of >95% to detect a minimum reduction
of 1 day in hospital stay between laparoscopic surgery and open surgery, a 1 day reduction
between fast track care and usual care, and a 80% power to detect the same difference
between the combination of fast track care with laparoscopic surgery and current treatment. A
much larger difference can be expected between the treatment groups for instance open &
standard care compared with fast track & laparoscopy (> 4 days). In order to obtain results
with adequate precision we have calculated group size using a difference of 1 day rather than
4 days.
With a group size of a hundred patients per arm it is possible to find a significant difference
(alfa=0.05, beta=0.1) of at least 10% in subscales of the SF-36, a validated Quality of life
Questionnaire, at two weeks after surgery (18-20). Liem et al (18) demonstrated 20-30%
differences in subscales of the SF-36 between laparoscopic versus open hernia repair 1 week
after surgery. Maartense et al. found a 10% difference in role physical and social function two
weeks after surgery comparing laparoscopic versus open ileocolic resection in a randomized
study from our institution (21).
Economic evaluation:
The marginal direct medical, non-medical and time cost differences will be calculated for the
four treatment strategies. These will include the additional costs of laparoscopy, of fast track
care, as well as the differences due to complications and readmissions.

Beleid bij colon chirurgie:
LAparoscopie en/of FAst track multimodaal management versus standaard zorg
Geachte heer/mevrouw,
Uw dokter heeft u gevraagd om mee te doen aan een onderzoek. Hierbij doen wij u informatie
toekomen met betrekking tot dit onderzoek. Doel van deze studie is te onderzoeken op welke manier
het beste in uw geval de dikke darmtumor kan worden weggehaald. Chirurgisch gezien kan de operatie
als kijkoperatie(laparoscopisch) uitgevoerd worden of als een open operatie. Tevens kan de
voorbereiding op de operatie en begeleiding rondom de operatie standaard zijn of volgens een zgn.
versneld herstel programma.
Waarom dit onderzoek?
De twee grootste ontwikkelingen van de laatste jaren in de chirurgie van de dikke darm zijn enerzijds
de dikke darm operaties uitgevoerd als kijkoperatie anderzijds de zogenaamde versneld herstel
programma s rondom de operatie. Zowel de kijkoperatie als het versneld herstel programma rondom
de operatie lijken gepaard te gaan met een sneller herstel na de operatie en eerder ontslag naar huis.
Het is echter nog niet goed uitgezocht of de darmoperatie het beste als kijkoperatie gedaan kan
worden, of dat de patiënt een versneld herstel programma moet doorlopen of misschien wel een
combinatie van beiden. Aangezien beide ontwikkelingen grote invloed hebben op enerzijds de manier
van opereren, anderzijds op de manier waarop u begeleid wordt rondom de operatie is het belangrijk
dit in een wetenschappelijk onderzoek uit te zoeken.
Wat houdt het onderzoek in?
Als u besluit mee te doen met het onderzoek, zal het lot (randomisatie) bepalen of U enerzijds de
kijkoperatie of de open operatie krijgt, en anderzijds of U standaard wordt behandeld of het
versneld herstel programma zal doorlopen. U zult voor de operatie niet te horen krijgen of u de
kijkoperatie geloot heeft. Ook na de operatie zal uw buik met een groot verband worden afgedekt,
opdat u direct na de operatie niet weet of u de kijkoperatie of de open operatie heeft gehad. Ook de
verpleging en zaalartsen mogen dat nog niet weten. Op deze wijze beïnvloedt de grootte van de snee
niet het tijdstip waarop u naar huis mag. Pas als u kunt eten, zich zelf kunt verzorgen, uit bed bent en
de opvang thuis geregeld is, kan u naar huis. Bij ontslag zal het verband worden verwijderd.

22. In het versnelde programma wordt u veel uitgebreider dan normaal voorgelicht voor de operatie, krijgt
U tevens routinematig een ruggenprik als ondersteuning voor de narcose en als pijnstilling na de
operatie en zal u eerder uit bed moeten en eerder gaan eten na de operatie.
Zijn er risico s of bijwerkingen bij deelname aan het onderzoek?
Alhoewel in de meeste ziekenhuizen de dikke darm nog open wordt verwijderd en de zorg rondom de
operatie standaard is, zijn zowel de kijkoperatie als de versneld herstel programma s veilige
alternatieven gebleken. U loopt dus geen extra risico s bij deelname aan deze studie.
Welk voordeel zult u hebben van deelname aan dit onderzoek?
Door deelname aan de studie zult u optimale zorg krijgen, waarbij het niet uitmaakt welke techniek of
behandelschema u loot. Een onderzoeksverpleegkundige en het medisch team zal u naast de
gebruikelijke zorg nauwlettend volgen om uw herstel goed in kaart te brengen.
Is uw privacy gewaarborgd?
Tot uw persoon herleidbare onderzoeksgegevens kunnen slechts met uw toestemming door daartoe
bevoegde personen worden ingezien. Deze personen zijn medewerkers van het onderzoeksteam,
medewerkers van de Inspectie voor de Gezondheidszorg of bevoegde inspecteurs van een buitenlandse
overheid, en leden van de Medisch Ethische Toetsingscommissie. Inzage kan nodig zijn om de
betrouwbaarheid en kwaliteit van het onderzoek na te gaan. Onderzoeksgegevens zullen worden
gehanteerd met inachtneming van de Wet Bescherming Persoonsgegevens en het privacyreglement
van uw ziekenhuis. Persoonsgegevens die tijdens deze studie worden verzameld, zullen worden
vervangen door een codenummer. Alleen dat nummer zal gebruikt worden voor studiedocumentatie, in
rapporten of publicaties over dit onderzoek. Slechts degene, die de sleutel van de code heeft (de
onderzoeker of de behandelend arts) weet wie de persoon achter het codenummer is. De gegevens
worden bewaard gedurende het onderzoek en na afloop vernietigd.
Voor dit onderzoek is goedkeuring verkregen van de Medisch Ethische Toetsings Commissie van het
Academisch Medisch Centrum. De voor dit onderzoek geldende internationale richtlijnen zullen
nauwkeurig in acht worden genomen.

23. Verzekering
Het AMC heeft een verzekering afgesloten waaruit eventuele schade als gevolg van het onderzoek
betaald kan worden.(Voorwaarden zie bijlage).
Consultatie
Wilt u over uw deelname graag spreken met een arts die niet bij het onderzoek betrokken is, dan kunt u
contact opnemen met dr. O.R.C. Busch, chirurg (020-5669111; sein 62770).
Werkt u mee aan het onderzoek? U beslist zelf.
Uw medewerking aan dit onderzoek is vrijwillig. Als u toestemming geeft om aan het
onderzoek mee te doen, heeft u te allen tijde de vrijheid om op die beslissing terug te komen.
U hoeft hiervoor geen verklaring te geven. Mocht u besluiten af te zien van deelname dan
heeft dit geen consequentie voor de verdere behandeling en de dokter-patiënt relatie. In het
geval u afziet van deelname aan de studie is de standaardbehandeling in dit ziekenhuis een
operatie.
Indien U besluit deel te nemen dan zal uw huisarts/behandelend arts met uw toestemming
hierover geïnformeerd worden.

24. VERZEKERING
Overeenkomstig de Wet medisch-wetenschappelijk onderzoek met mensen heeft het VUMC
voor medisch-wetenschappelijk onderzoek een verzekering afgesloten die door het onderzoek
veroorzaakte schade door dood of letsel van de proefpersoon dekt.
* Dit betreft schade die zich tijdens of binnen vier jaar na de deelname aan onderzoek
openbaart en gemeld is binnen 4 jaar na beëindiging van de deelname aan het onderzoek.
* Het bedrag waarvoor de verzekering is afgesloten bedraagt 450.000 per proefpersoon, met
een maximum van 3.500.000 voor het gehele onderzoek en 5.000.000 voor schade ten
gevolge van medisch-wetenschappelijk onderzoek die per verzekeringsjaar wordt gemeld.
De verzekering biedt dekking
* voor schade tengevolge van de verwezenlijking van de aan deelname aan het
wetenschappelijk onderzoek verbonden risico's waarover men niet schriftelijk is ingelicht;
* voor schade tengevolge van de verwezenlijking van de risico's waarover de deelnemer
wél is ingelicht, maar die zich in ernstiger mate voordoet dan is voorzien;
* voor schade tengevolge van de verwezenlijking van de risico's waarover de deelnemer
wél is ingelicht, maar die zeer onwaarschijnlijk werd geacht.
De verzekering biedt geen dekking voor schade
* voor schade die het gevolg is van het uitblijven van een vermindering van de
gezondheidsproblemen van de proefpersoon, dan wel het gevolg is van de verdere
verslechtering van de gezondheidsproblemen, indien de deelname aan het
wetenschappelijk onderzoek plaatsvindt in het kader van de behandeling van die
gezondheidsproblemen;
* voor schade door aantasting van de gezondheid van de proefpersoon waarvan
aannemelijk is dat deze zich ook zou hebben geopenbaard wanneer de proefpersoon niet
aan het onderzoek had deelgenomen;
* voor schade tengevolge van deelname aan medisch-wetenschappelijk onderzoek
waarbij in de kring van beroepsgenoten gebruikelijke handelingen op het gebied van de
geneeskunst met elkaar worden vergeleken en aannemelijk is dat de schade het gevolg
is van de toegepaste handelingen;
* voor schade die zich bij een nakomeling van de proefpersoon openbaart als gevolg van
een nadelige inwerking van het onderzoek op de proefpersoon of de nakomeling;
* voor schade die het gevolg is van het niet of niet volledig opvolgen van aanwijzingen
en instructies door de proefpersoon, indien de proefpersoon daartoe althans in staat is.
De verzekering dekt uitsluitend de schade van natuurlijke personen.
De dekking van specifieke schades en kosten is tot bepaalde bedragen beperkt.
Om aanspraak te kunnen maken op schadevergoeding dient de proefpersoon in geval
van vermeende schade als gevolg van het onderzoek dit te melden aan:

Verpleegkundig Klinisch Zorgpad Versneld Herstel Programma Fast Track / LAFA
studie
Dit zorgpad beschrijft de opname van dag -1 t/m dag 3 van patiënten die Fast Track
perioperatieve zorg krijgen. De belangrijkste verpleegkundige handelingen/interventies van
dit versneld herstel programma zijn chronologisch beschreven. Als er van het programma
wordt afgeweken is het belangrijk de verandering en de motivatie te beschrijven in de
rapportage.
Het programma berust op de volgende criteria:
1. Verpleegkundig spreekuur
2. Preop® voeding pre-operatief
3. Thoracale epiduraal
4. Post-operatief geen maagsonde
5. Post-operatieve mobilisatie
6. Orale intake post-operatief
7. Belafspraak na ontslag
Het is belangrijk patiënten gedurende het programma op positieve wijze te stimuleren en te
ondersteunen. Alles gebeurd op geleide van de patiënt!
Patiënten krijgen bij opname een helder vloeibaar dieet tot 2 uur voor de operatie
Direct post-operatief krijgen patiënten een normaal dieet. De avondmaaltijd zal geen
warme maaltijd zijn maar veelal yoghurt, vla en een boterham. Tussen de maaltijden door
krijgen patiënten drinkvoeding aangeboden, minimaal 3 pakjes per dag.
Bij sensibiliteitsstoornissen t.g.v. epiduraal: patiënt niet laten lopen (wel in de stoel!), SP of
CAD niet verwijderen. Anesthesie is verantwoordelijk voor de epidurale pijnstilling.
Pijnstilling bij epiduraal: 4 dd Paracetamol 1000 mg (standaard) per os. Geen Diclofenac
tijdens epiduraal. Bij de proefstop ( dag 2 om 6.00) Paracetamol 1000 mg en Tramal tot 100
mg geven. Na het verwijderen van de epiduraal kan Diclofenac (in combinatie met
Nexium/Pantozol), 3 dd 50 mg, gestart worden en Tramal 50 mg doorgegeven.
Het beëindigen van de epiduraal op de 2e dag post-operatief is protocollair! Uiteraard is de
proefstop afhankelijk van de pijnscore en de situatie van de patiënt en kan dus uitgesteld
worden.
