1. Creating a Google Form
*note – you MUST have a Google account in order to create a form.
1. Go to Google Drive (it also may be referred to as Forms)
2. Enter the question you’d like to ask and any instructions/details you’d like to include pertaining to
the inquiry.
3. At the bottom of the screen you notice a link that indicates “You can view the published form here:”
2. SAVE THIS LINK as it will take you to the published form, as seen below.
The form you are creating will look like this:
3. 4. You have the option to email this form:
5. You have the option to embed the form to your website/blog/etc..
4. Code is displayed, which can be cut+pasted to your website/blog/etc….
*note – embedded code is not the same as the url. The embed code is the html code that you use to post
the survey (video, picture, other misc) to your site. The url is just the address of where the video is
located.
7. 7. The results of the form are stored on to your Google account.
Note – this only displays the database as it aggregates from #6. In order to modify to original form you
MUST have the URL #3.
