הפליטות משריפת מזוט.
ניקל – מתכת אלרגנית. חשיפה לניקל דרך האוויר מעוררת התקפי •
אסטמה,
ברונכיטיס כרונית, ירידה בתפקודי ריאות, נזק לכבד ולכליות. נמצא
קשר בין חשיפה לניקל לבין סרטן ריאות והסינוסים.
חלק מהתרכובות של ניקל נחשבות למסרטנות וודאיות.
ה- EPAבארהquot;ב קבע כי אבק ניקל שנפלט מתהליכי הפקה של ניקל
וכן גם ניקל סובסולפיד הם מסרטנים עבור בני אדם. )(ATSDR

ניתן להעריך שמאות אלפי התושבים בצפון נחשפו
למתכות ותרכובות אורגניות דרך האוויר כתוצאה מפליטות
בתי הזיקוק, המפעלים הפטרוכימיים ותעשיות כימיות

16. חיזוק להשערה זו ממחקרים במודלים של
חיות
קיימים ממצאים רבים אשר תומכים בכך שחשיפה לחלקיקים קטנים מזיקה
ללב ומעודדת מצבי דלקת, שהם התחלה של תהליך טרשתי.
דלקת - חולדות ועכברים קשישים מועדים יותר לפתח דלקת כתוצאה מזיהום בחלקיקים: בחשיפה לחלקיקי מתכת
)ברזל( ייצרו תאי החולדות המבוגרות כמות ציטוקינים גבוהה ב-% 03-05 ממה שייצרו חולדות צעירות יותר.
בחשיפות משולבות לחלקיקים ultrafineו- LPSהיתה סינרגיה בחולדות קשישות.
השפעות על הלב: לחולדות שנחשפו לזיהום אוויר מניו יורק היה שינוי בתמונת הא.ק.ג: היתה להם עלייה מובהקת
בתדירות של .supraventricular arryhthmiasלניסוי זה היתה מתודולוגיה טובה מאחר והביקורת של כל חולדה היתה
היא עצמה כשנחשפה לאוויר לא מזוהם. אצל אותן החולדות כשנחשפו לזיהום ב 2 – SOלא נמצאו שינויים בא.ק.ג מה
שמעיד כי שימוש ב 2 SOכפרוקסי של חשיפה להשפעות על הלב - איננו תורם רבות.
– MIבמודלים של עכברים שחולים ב – MIנמצא כי חשיפה לחלקיקי Oil Fly Ashהביאה לעלייה באי - סדירויות קצב
הלב.
- ROSתרכובות חמצן פעילות-מחקרים בחיות הראו שחומר חלקיקי מעודד יצירת ROSים שהם מעודדים דלקת
ןמחזקים את התגובה לאלרגנים. עובדה זו עשויה להסביר מדוע כשיש עלייה בזיהום אוויר חלקיקי, יש הסובלים
מהתקפי אסטמה חמורים. כמו כן תרכובות אלו קשורות גם להשפעה על הלב.

17. חשיפה לטווח קצר – נמצאו השפעות על הלב
חשיפה קצרה למזהמי אוויר חלקיקיים )5.2 PMומטה( ברמות גבוהות גורמת ל:
א. עלייה בתמותה באופן מובהק, באשפוזים, בהתקפי אסטמה ועוד.
ב. הפרעות קצב לב, שינויים ב QTCוב ,HRVאיסכמיה, ועלייה ברמת סמנים
של סטרס ושל דלקת - כגון 6 ILו1 ,ICAMאשר מעלים את קרישיות הדם.
כתוצאה מכך עולה הסיכון לחסימת כלי דם.
ג. שינוי ברמות של מולק' – CD40Lכתוצאה מכך עולה הסיכון לחסימת כלי דם
ולארועי לב.

בשנת 2991 פרסמו החוקרים דquot;ר גארי גינזבורג ופרופ' תד טולצ'ינסקי
מחקר בו השוו נתוני תמותה )מסיבות שונות( לפי נפות. החוקרים
חישבו – SMRכלומר הנתונים מתוקננים לגיל, מין, ארץ מוצא ודת,
ונבדקו שלוש תקופות שונות:
א. 9691-8791 – חושבו SMRים עבור יהודים בלבד.
ב. 3891-6891 – חושבו SMRים עבור יהודים ולא יהודים.
ג. 7891-4991 – חושבו SMRים עבור יהודים בלבד.
לאור הממצאים שהוצגו קודם לכן, רציתי לבחון מחדש תוצאות
מחקר זה בעזרת ) GISממquot;ג( עבור המחלות א שר באופן מובהק עודף
תמותה מהן נמצא כקשור בזיהום אוויר חלקיקי מסוג 5.2. PM
העבודה נעשתה במסגרת פרוייקט בקורס quot; ,quot;GISבהנחייתו של דquot;ר
בוריס פורטנוב.

29. האם יש קורלציה עם נתוני זיהום?
כרגע יש בידנו רק נתוני זיהום עבור 01 PMברמה
הארצית. כמו כן הנתונים הם עבור שנים מאוחרות
יותר )1002-4002( מהמוצג כאן.
מאחר ועבור תמותה ממחלות לב נמצאו הקשרים
הטובים ביותר עם חלקיקים מסוג 5.2 ,PMואלו
אינם נמדדים באופן קבוע בכל תחנות הניטור, לא
ניתן היה לבדוק בעבודה זו קורלציה בין המפות.
הערה: דווקא במפות של 01 PMמוצג אזור הדרום
כמזוהם. זאת מאחר ובין היתר נכללים ב01PM
חלקיקים של אבק מדברי, אשר ) .Laden et al
2002 מצאה אותם כלא- משפיעים על תמותה
ממחלות לב. כמו כן היא מצאה קשר בין פליטות
משריפה פחם לתמותה.
למעשה, מה שחשוב, הוא אופי החלקיקים הנפלטים,
מתכות וחומרים אורגניים מסכנים את בריאות הציבור.

30. מסקנות
א. בשנים שנבדקו ניתן לראות הבדלים בתמותה לפי נפות. ההבדלים משמעותיים, ומגיעים לעד עשרות
אחוזים מעל לממוצע הארצי. לפי הידוע מהספרות הבינquot;ל, הנתונים יכולים לרמז על מקור זיהום סביבתי
מקומי בחלקיקים שקוטרם 5.2 מיקרון ומטה.
ב. סביר להניח שהבדלים אלו אינם נובעים ממוצא שונה, מאחר והחוקרים ניטרלו הבדלים אתניים בעזרת תקנון
)אשכנזים, יוצאי צפquot;א וכו'(. כמו כן ההסבר לא יכול להיות גנטי, מאחר וגנטיקה בלבד איננה מסבירה עודפי
תחלואה כה גדולים במחלות אלו.
ג. ממצאי הספרות שהוזכרו מספקים הסבר אפשרי לתחלואה במחלות לב באזורים הללו.
ד. יתכן וחלק מההבדל הנצפה נובע מתעסוקה, אך לא כולו, מאחר ועודפי התמותה בנתונים גולמיים עוברים את
מספר העובדים בתעשיות הפטרוכימיות באזור. כמו כן, מאחר ולא משוחררים מטעם המפעלים נתונים על
תחלואה עובדים, לא ניתן לבדוק השערה זו לעומק.
ה. לא סביר שהעלייה ברמות התמותה בנפות הצפוניות מוסברת על ידי אי נגישות לשרותים רפואיים
מאחר וקיימים בתי חולים רבים בנפות הצפוניות )כרמל, רמבquot;ם, בני ציון, פוריה, צפת וכו'(
אשר המרחק אליהן קרוב מהמרחק לסורוקה בנגב. מה גם שבנפות המרוחקות אין עומסי תנועה.
ו. יש צורך בהמשך מחקר וכן הצלבה עם נתוני פליטות 5.2 PMונתונים פליטות מתכות ומזהמים אורגניים
על מנת לברר לעומק את פשר עודפי התמותה.
במקביל, לאור ממצאי המחקרים הבינquot;ל, יש צורך מיידי להקטין את פליטת המתכות והמזהמים
האורגניים באזור חיפה והצפון וחדרה, על מנת למנוע המשך תמותה עודפת ממחלות לב ריאה.

Myocardial infarction (MI) is the rapid development of myocardial necrosis caused by a critical :Background •
imbalance between the oxygen supply and demand of the myocardium. This usually results from plaque rupture
with thrombus formation in a coronary vessel, resulting in an acute reduction of blood supply to a portion of the
.myocardium
Although the clinical presentation of the patient is a key component in the overall evaluation of the patient with MI, •
many events are either “silent” or are clinically unrecognized, evidencing that patients and physicians often do not
recognize symptoms of a MI. The appearance of cardiac markers in the circulation generally indicates myocardial
.necrosis and is a useful adjunct to diagnosis
Cardiac markers also help to categorize MI, which is considered part of a spectrum referred to as acute coronary •
syndromes that includes ST-elevation MI (STEMI), non–ST-elevation MI (NSTEMI), and unstable angina. This
categorization is valuable because patients with ischemic discomfort may or may not have ST-segment elevations
on their electrocardiogram. Those without ST elevations will ultimately be diagnosed with NSTEMI or with unstable
angina based on the presence or absence of cardiac enzymes. Additionally, therapeutic decisions such as
administration of an intravenous thrombolytic or performing percutaneous coronary intervention (PCI) is often
.made based on this categorization
The most common cause of MI is narrowing of the epicardial blood vessels due to :Pathophysiology •
atheromatous plaques. Plaque rupture with subsequent exposure of the basement membrane results in platelet
aggregation, thrombus formation, fibrin accumulation, hemorrhage into the plaque, and varying degrees of
vasospasm. This can result in partial or complete occlusion of the vessel and subsequent myocardial ischemia.
Total occlusion of the vessel for more than 4-6 hours results in irreversible myocardial necrosis, but reperfusion
.within this period can salvage the myocardium and reduce morbidity and mortality
Nonatherosclerotic causes of MI include coronary vasospasm as seen in variant (Prinzmetal) angina and in •
patients using cocaine and amphetamines; coronary emboli from sources such as an infected heart valve;
occlusion of the coronaries due to vasculitis, or other causes leading to mismatch of oxygen supply and demand,
such as acute anemia from GI bleeding. Finally, MI induced by chest trauma has also been reported, usually
.following severe chest trauma such as motor vehicle accidents and sports injuries
