This SCHOLARSHIP GRANT AGREEMENT made and executed in the place and date
mentioned herein below by and between:

St. Dominic Academy Batch ’78 Alumni Association, Inc. (‘SDA Batch ’78’), a non‐
ALUMNI ASSOCIATION INC.
profit organization duly registered under Philippine laws, with its business address at
Poblacion, Pulilan, Bulacan and represented by its Scholarship Committee Chairman
or its duly recognized representative, ______________________________________,
hereinafter referred to as the GRANTOR;

__________________________________ (name of student), a minor, with residence
and postal address at ________________________________________________,
and herein lawfully represented by his/her parents or legal guardians,
____________________________________________________, thereafter called
the GRANTEE.
WHEREAS, the GRANTOR has established the SDA Batch ’78 Scholarship Grant that
aims to provide financial support [‘Scholarship’] to two (2) deserving secondary
school freshmen enrolled at the night school of St. Dominic Academy [‘SDA’] in
Poblacion, Pulilan, Bulacan;
WHEREAS, after careful screening by the Scholarship Committee of the GRANTOR
among possible grantees, herein GRANTEE was selected based on the guidelines the
GRANTOR has established.
WHEREAS, the GRANTOR awards and the GRANTEE accepts the said scholarship grant
for the school years _________ to _________.

In consideration of the scholarship grant offered by the GRANTOR to the GRANTEE,
the parties hereby agree to the foregoing terms and conditions of this Scholarship

1. GRANTEE agrees to comply with the rules, regulations, policies and procedures
of both SDA and the GRANTOR. For this purpose, the GRANTEE shall be provided the
General Guidelines of the Night School Scholarship [‘Guidelines’] for his/her
continued perusal and which Guidelines form part of this Agreement.

2. While the GRANTEE agrees to attend classes regularly, and to complete
classroom and academic assignments, he/she is further required to formally
submit to the GRANTOR, a certified true copy issued by SDA of his/her quarterly
report cards for the GRANTOR’s monitoring purposes.

2.1. Should the GRANTEE opt to discontinue his/her studies, or his/her

grades fall below 80% for a school year, or that his/her school
attendance falls below the required number of days attended, this
Agreement is automatically terminated.
3. GRANTEE understands and agrees that the duration of the scholarship is five
academic years and that in no instance or under any circumstances will the
scholarship be extended.
4. Under this Agreement, the GRANTOR will provide to the GRANTEE the following:
4.1. Monthly tuition fees
4.2. Textbooks & school supplies
4.3. School shoes & uniforms (both for regular and physical education)
4.4. Transportation & summer allowances
4.5. Other school‐related miscellaneous and incidental expenses
5. The details of the above financial grant are provided for in the Guidelines. While
the amounts of financial assistance extended are determined at the beginning of
the school year, the GRANTOR has the option to modify the same at any given
time.
6. Personal spending money and other expenses not school‐related are for the
account of the GRANTEE alone.
7. Medical, dental, and/or optical expenses incurred by the GRANTEE by reason of
his/her attendance to school or otherwise likewise do not form part of the

8. GRANTEE agrees to indemnify and hold GRANTOR and SDA harmless of and

from any and all demands, claims, liabilities or actions of any kind whatsoever,
excepting only payments agreed by the GRANTOR to be made by it in Paragraph

9. GRANTEE further understands and agrees that this Agreement granted may be
terminated for cause, other than the reasons under Paragraph 2.1 at the sole
discretion of the Scholarship Committee of the GRANTOR.
