Kienbock's disease is an avascular necrosis of the carpal lunate bone, most common in ages 15-40. The cause is unknown but repetitive trauma is thought to compromise the lunate's blood supply. It progresses through four stages from ischemia to bone collapse and fragmentation. Symptoms include dorsal wrist pain aggravated by axial loading. Imaging shows increasing sclerosis and collapse over time. Treatment ranges from casting early on to various surgical options like ulnar lengthening or lunate replacement/excision depending on the stage.
