This 68-year-old male presented with pain and swelling in his left leg for one day along with altered mental status and fever. On examination, he had diffuse swelling, brawny edema, and bullae on his left leg. Lab results showed signs of sepsis including elevated white blood cell count and abnormal blood gases. Gram stain of fluid from a bulla showed mixed aerobic and anaerobic organisms. The most appropriate therapy would be penicillin, clindamycin, and debridement to treat the septic shock likely caused by necrotizing fasciitis from group A streptococcus infection.
