Mosaic provides supports to individuals with

developmental disabilities and to senior citizens
in more than 100 communities across 14 states
including Arizona, Colorado,Connecticut,
Delaware, Illinois, Indiana, Iowa, Kansas,
Nebraska, New Mexico, Tennessee, Texas, Utah
and Wisconsin.
Mosaic operates two independent senior living

communities in Wisconsin and Iowa.
8. Heritage
Mosaic was formed on July 1, 2003
when Bethphage and Martin Luther
Homes consolidated their ministries
into one organization that serves
thousands of people nationally and
internationally.
9. Worldwide Impact
Mosaic now works with international partners
in Great Britain, Germany, Norway, Latvia,
Romania, Russia, Tanzania, Kenya, and the
Dominican Republic.

Case Management: Coordination of
services and supports for people who
might receive other services or supports
from Mosaic or another service provided.
14. Supports We Provide
Children's Services:
Services provided to
people under the age of 21.
Supports may include residential services,
supported/intermittent, host homes, day services
(transition),special education, daycare, or early
intervention.
15. Supports We Provide
Day Services:
Any vocational services,
supported employment,
job coaching, community
participation or habilitation
training.
16. Supports We Provide
Residential: Supports are provided 24
hours/day and 7 days/week.
We provide affordable and accessible housing. Both
ICF/MR and waiver residential settings are included.
Generally these services are provided in an agency
leased or owned-facility.
17. Supports We Provide
Supported Living: Supports are provided fewer
than 24 hours per day and can be provided in a
home owned or leased by the client of the
agency.
Supports can vary from a few hours a week to
several hours a day. These services are provided
through a waiver facility and usually are based
upon the needs of each person.

Host Homes: Services provided by an
independent contractor while individual lives
in that person's home.
Individuals served in host home settings have a range
–
of care needs, from total care (24 hours a day, seven
days a week) to limited care (those who are semi-
independent and employed in the community).

Affordable and Accessible Housing:
Because most people with developmental
disabilities are not able to find competitive
employment, poverty is a serious issue.
Mosaic partners with state and federal
agencies to create affordable housing that
is accessible for all people. Residents must
meet federal income guidelines to qualify.
