TRIALS RANDOMIZADOS: The first, by Lo et al (29), compared survival outcomes with chemoembolization versus symptomatic management, with 40 patients per group. One, 2-, and 3-year survival rates in the study group were 57%, 31%, and 26% compared with 32%, 11%, and 3% in the control group ( P .02). On univariate analysis, chemoembolization remained a significant predictor of survival (odds ratio, 0.49; P .006). The second study, by Llovet et al (30), included 112 patients in three arms and compared outcomes with chemoembolization versus embolization alone versus symptomatic treatment. The trial was stopped when a significant survival benefit was demonstrated with chemoembolization (survival rates, 82% at 1 year and 63% at 2 years) versus symptomatic treatment (63% at 1 year and 27% at 2 years; P .009). At the time the trial was halted, there was not a survival benefit identified with embolization alone (75% at 1 year and 50% at 2 years) versus symptomatic treatment, although the difference may have reached significance with continuation of the trial. The only variable associated with prolonged survival was assignment to the chemoembolization group (odds ratio, 0.45; P .02). Publicações de 2002
A French multicenter trial (31) of 127 patients with more typically advanced disease, such as is diagnosed in unscreened populations in the United States (62% Okuda stage 1 or 2 disease), showed survival rates in the chemoembolization arm of 64% and 38% at 1 and 2 years, with survival rates in a matched concurrent untreated control arm of only 18% and 6%, respectively ( P .0001). A more recent trial compared outcomes of 110 patients with HCC treated with chemoembolization versus 83 patients treated with symptomatic management (32). The two groups were matched by their demographic characteristics and clinical characteristics of disease. The group treated with chemoembolization had a 54% survival rate at 2 years, compared with 26% in untreated control subjects. Mean survival in the treatment group was 26 months, compared with 10 months for the control group ( P .0001). These and several similar reports from the 1990s consistently show a two- to three-fold increase in median survival after chemoembolization compared with untreated control subjects, with a high degree of statistical significance.

In the combined and monotherapy groups, respectively, 64.6% and 49.2% of patients had vascular invasion, 87.8% and 91.1% had extrahepatic metastasis, and 54.3% and 47.1% had both. At univariate and subsequent multivariate analyses, additional TACE was an independent predictor of favorable TTP and OS
Median follow-up period of 21.4 weeks. The combined use of sorafenib and TACE was generally well tolerated and significantly improved OS and TTP compared with TACE alone in patients with intermediate or advanced HCC. Further studies are warranted to confirm the safety and efficacy of this combination therapy. Significant prognostic factors for OS by multivariate analysis included the use of sorafenib, Barcelona Clinic Liver Cancer stage, metastasis/vascular invasion and Child-Pugh score.
TACE with DC bead® (n=60) to controls who received cTACE (n=69). The primary end points were treatment response and treatment-related adverse events. The secondary end point was time to progression. DC bead® treatment resulted in a significantly better treatment response and longer time to progression than cTACE (p<0.001 and 0.038, respectively). However, there was no statistically significant difference in liver toxicity between the DC bead® and cTACE group (p>0.05). TACE with DC bead® showed better treatment response and delayed tumor progression compared with cTACE. There was no significant difference in hepatic treatment-related toxicities. DC bead® TACE thus appears to be a feasible and promising approach to the treatment of HCC.
The DEBDOX had a significant (p<0.05) advantage in objective response in the more advanced patients (defined as Child-Pugh B, ECOG 1, recurrent disease and bilobar disease; p=0.038) and overall disease control in the more advanced patients (p=0.026). The DEBDOX Bead was also found to have a highly significant (p<0.01) advantage in the reduction of doxorubicin associated side effects (p=0.0001) in all patients.
percutaneous radiofrequency ablation (RFA) can be as effective as surgical resection in terms of overall survival and recurrence-free survival rates in patients with small HCC, and a retrospective comparative study reported 1-, 3-, and 5-year overall survival rates and recurrence-free survival rates significantly better in patients with central HCC measuring 2 cm or smaller treated with RFA than in those treated with surgical resection. However, RFA is size-dependent, so at present the need to achieve an adequate safety margin around the tumor limits to about 2 cm the diameter of the nodules that can be ablated with long-term outcomes comparable to or better than surgical resection. The main goal of the next technical developments of the thermal ablation systems should be the achievement of larger ablation areas with a single needle insertion.
Seven randomized-controlled trials were included. Meta-analysis showed that RFA plus TACE significantly improved the survival rates of patients with HCC at 1 and 3 years (for the one-survival rate, fixed-effects OR=2.71, 95% CI 1.65-4.43, P<0.0001; for the three-survival rate, fixed-effects OR=2.27, 95% CI 1.57-3.27, P<0.0001) compared with RFA alone. Subgroup analyses by tumor size showed that RFA plus TACE significantly improved the survival rates at 1, 3, and 5 years compared with RFA alone in patients with HCC larger than 3 cm; however, there was no advantage of TACE plus RFA over RFA alone for patients with HCC smaller than 3 cm. The quality of evidence was high for the 1-year survival rate, the 3-year survival rate, and major complications
