14. Please put the date of request. Neatly print name and address of Admissions Office. If multiple campuses, please specify which one you are applying to! PLEASE specify application type. We need to know this so we are aware of the deadlines. Include due date! NOT ASAP!!! If requested will be sent in with transcript. Check application to see if there is a Secondary School Report. If so, please print out and submit to counselor with transcript request.
