La injuria primaria se refiere a la lesión estructural y funcional que desarrolla un tejido al soportar un trauma Este tejido conforma un área que se ha denominado umbra y por su definición implica tejido perdido o que irremediablemente se va a perder, a la luz del manejo médico actual1.13* La cantidad de tejido en la umbra, sin embargo, frecuentemente no alcanza a ser el suficiente para generar un mal desenlace, en un paciente en la mayoría de los casos de TCE aún severo. 14 Con esto se debe entender que en muchos casos , la causa del los malos desenlaces es la extensión posterior del área de tejido lesionado . 13 Este concepto es trascendental al considerar el manejo de estos pacientes, pues la percepción de muchos, en nuestro medio latinoamericano, es que un paciente con TCE severo tiene un pronóstico sombrío, por lo que no se justifica hacer esfuerzos especiales que se suponen son fútiles.
Skull Fractures: Skull fractures may be seen in the cranial vault or skull base, may be linear or stellate, and may be depressed or nondepressed. The presence of a skull fracture implies that a large amount of force was transmitted to the patient’s head. A linear vault fracture increases the likelihood of the presence of an intracranial hematoma. Epidural Hematomas: Epidural hema tomas are relatively uncommon, being present in 1% of all head-injured patients and in 10% of those who are comatose. Subdural Hematomas: Subdural hematomas are more common than epidural hematomas, occurring in approximately 30% of severe head injuries. They result most frequently from tearing of a bridging vein between the cerebral cortex and a draining venous sinus. Intracerebral Hematomas: Intracranial hemorrhage occurs commonly in association with moderate and severe head injuries and usually produces mass lesions. The majority of lesions occur in the frontal and temporal lobes. Many intraparenchymal hematomas may be delayed, appearing on the CT scan 24 h after the initial insult.8– Diffuse Axonal Injury: Diffuse axonal injury is caused by shearing forces affecting axons that traverse large areas of the brainstem, leading to dysfunction of the reticular activating system.11 It is believed that axons are not torn at the moment of injury but rather undergo sequential, focal changes that lead to swelling and disconnection over multiple hours after injury.12,13 As a consequence of this disconnection, the downstream disconnected fibers degenerate leading to diffuse deafferentation of target sites.12–14 Evidence suggests that the traumatic axon injury results from damage to the axolemma,
Together, these events lead to membrane degradation of vascular and cellular structures and ultimately necrotic or programmed cell death (apoptosis).

Normal human values for CPP are between 70 mm Hg and 100 mm Hg. However, as a result of autoregulation, CBF remains relatively constant when CPP is between 40 mm Hg and 140 mm Hg (Fig 4).34 This phenomenon is due to changes in cerebrovascular resistance probably brought about by a local effect of hydrogen ions on cerebral vessels.34 Thus, low flow states leading to hypoxia or hypercapnia result in an acidosis that causes cerebral vasodilation and increased blood flow. Chronic hypertension shifts the autoregulation curve to the right, making hypertensive patients susceptible to ischemia at a CPP normally well tolerated by healthy subjects (Fig 4). Cerebrovascular autoregulatory mechanisms are disrupted following head trauma, with CBF dependent largely on the CPP. While earlier studies and recommendations centered on the importance of the ICP per se in the head-injured patient,35 current guidelines emphasizes the importance of the CPP.36–38 The guidelines proposed by the Brain Trauma Foundation recommend that the CPP should be maintained at a minimum of 70 mm Hg in the brain-injured patient, although the exact target number and methodology for achieving that target remains controversial.

Cuando estos mecanismos se han agotado, cualquier aumento del volumen de la lesión produce ascensos marcados de la PIC.
Los aumentos de la PIC tienen un impacto enorme sobre el flujo sanguíneo cerebral, ya que por la rigidez del cráneo, la presión arterial (PA) existente en los vasos intracraneanos, no sólo debe vencer la resistencia vascular cerebral, sino la PIC en caso de estar aumentada. Se define entonces la presión de perfusión cerebral (PPC), que es el resultado del gradiente entre la presión arterial media (PAM) y la PIC. El aumento de la PIC, por encima de 10-15 mmHg, que son sus valores normales, incide sobre una reducción del gradiente de presión disponible para perfundir el parénquima cerebral. Cualquier alza de la PIC, requeriría entonces un aumento de la PA para mantener el flujo sanguíneo cerebral (FSC), de no haber cambios en la resistencia vascular cerebral, (RVC). Por ello, es posible lograr supervivencia, aún en algunos casos con elevación de PIC a valores como 40 mmHg.2 A su vez, un incremento de la PA puede aumentar el volumen sanguíneo cerebral y, por lo tanto, acrecer la PIC, pero el mayor FSC se refleja en vasoconstricción, por lo que se atenúa el aumento de PIC mediado por el incremento de PA y se deshace el círculo vicioso, que hay que evitar para prevenir un desenlace adverso, que es lo descrito por Rosner. 3 Sin embargo, hay que resaltar que en las fases iniciales del trauma es trascendental mantener este gradiente, ya que aún sin incrementos de la PIC, es posible que ocurran mecanismos de vasoespasmo que aumentan la RVC y hacen que incluso un gradiente normal pueda ser insuficiente para mantener el FSC. El alza de la RVC además es la responsable de los aumentos de la PIC y del FSC disminuído, que se han documentado en las fases iniciales de los pacientes con TCE severo, y que se traducen en isquemia crítica, que le da el “coup de grâce” al tejido circundante a la lesión estructural (o lesión primaria), que se conoce como la penumbra.4 Compensacion aumentado la extraccion de o2 de la sangre hasta 23

[10] suggests a target PaO2 of 55–80mmHg or SpO2 of 88–95%. The Brain Trauma Foundation guidelines [1] recommend avoidance of hypoxia (PaO2 <60mmHg), placed on 100% oxygen, with the inspired fraction of oxygen only titrated down once the patient has been transferred to the ICUs. Aggressive hyperventilation (Paco2 of 25 mm Hg) has traditionally been considered a cornerstone in the management of TBI because it is causes a rapid reduction in ICP. However, hyperventilation reduces ICP by causing cerebral vasoconstriction with a subsequent reduction in CBF. Skippen and colleagues,27 using xenonenhanced CT and CBF studies, demonstrated a 2.5-fold increase in the number of regions of brain ischemia in children with TBI who were hyperventilated. A high fraction of inspired oxygen may, however, promote the formation of reactive oxygen species and increase lipid peroxidation. While it has been suggested that positive end-expiratory pressure and modes of ventilation that increase mean intrathoracic pressure be avoided in patients with elevated ICP, fter the establishment of an airway and ventilation, the restoration of BP and normal circulating volume is of utmost importance. According to the Brain Trauma Foundation guidelines for the management of severe head injury, a mean arterial pressure of 90 mm Hg should be targeted; this was chosen based on attaining cerebral perfusion pressures 70 mm Hg.1 These guidelines use 20 mm Hg as the threshold for intracranial hypertension. 1

Amino-steroids TBI causes mitochondrial dysfunction, which in turn produces oxygen free radicals which cause membrane lipid peroxidation, leading on to membrane dysfunction. Lazaroids are steroid derivatives, which inhibit lipid peroxidation but do not have the receptor-dependent steroid side effects. Pre-clinical studies in rats, mice, and cats demonstrated efficacy of tirilazad in reducing mortality and morbidity after head trauma. Human Phase III trials have however been disappointing and there is no evidence of benefit (or harm) from the administration of tirilazad 10 mg kg21 every 6 h for 5 days, to patients with TBI, GCS 3–12 starting within 4 h of injury.

Conventionally, invasive ICP monitoring is used if GCS is 8. This is usually deferred for patients scheduled for immediate craniotomy. In those patients 426 E. Bedell, D.S. Prough / Anesthesiology Clin N Am 20 (2002) 417–439 scheduled for non-neurosurgical procedures under general anesthesia (eg, extremity surgery), one should consider placement of ICP monitoring devices for those with GCS 12 as they are at risk for intraoperative deterioration, and strongly consider placement for those with an initial GCS 8.

Failure of cellular metabolism eventually leads to degradation of cell membrane phospholipids and release of glycerol into the brain extracellular fluid (ECF). Glycerol is therefore a useful MD marker of cell damage after TBI and the degree of the hypoxia/ischaemia-induced elevation of MD glycerol may be dramatic, with 4- or 8-fold increases recorded in severe or complete ischaemia, respectively.51 Cerebral MD glycerol concentrations are typically elevated in the first 24 h after severe TBI, presumably as a result of the primary injury, and then exponentially decline during the ensuing 3 days.7 Subsequent increases in MD glycerol concentration are associated with adverse secondary events45 and seizure activity.63 Increased concentrations of excitatory amino acids31 and reduced brainECF glucose concentrations66 may also predict or be associated with metabolic catastrophes occurring after acute brain injury. MD is becoming established as a tool to assist clinical decision-making during neurointensive care, such as management of CPP,41 guidance of hyperventilation,34 and the appropriateness of extensive surgical procedures.
