Editor's Notes

  1. 2000 gallons 0f 28% N lost when tank ruptured while filling.
  2. 1000 gallons of 28% N lost when tank fell through bottom of truck trailer.
  3. Friday night – closed down the road– mda showedup on Monday– Road commission brought in sand to soak up 28%
  4. Fiber optics line Miss Dig 50,000 to repair
  5. DEQ rec was to flush to wetland and remediate wetland
  6. Farm shop fire - farmer at the scene but did not remember that chemicals were stored in the adjacent bay until the bright yellow color started running out. Diked area and collected fire water. Could have used an emergency tube to remind him of pesticide storage concerns.
  7. Farmer owns front end loader and has a lime spreader digging up based upon visual/smell? Observation materials spilled would have covered 10 acres - therefore cover 10 acres with soil - labeled site Problem here in that while waiting for cleanup, applicator came back and sprayed remainder of this field - had to find another labeled site for land application
  8. Lime spreader - always WEAR ppe not just hang on to it
  9. We can crunch all the numbers we want to make sure that the material gets spread as appropriate but it comes down to equipment capabilities and that usually means the you spread it as thin as you can covering as much area as possible Watch out for flying rocks!
  10. Tender box that used to be on ………...
  11. ……… .. This truck Only 3 out of the 8 bolts actually held the box in place Rolled over on road while going around corner Speed? Bolts? Shifting load?
  12. Dry material easy to clean up - especially fertilizer as you really do not have to get it all In this case, the Fire Department wanted to get the road cleaned off before the clean up of the DRY material was complete Can see runoff path from water applied by FD - brooms and shovels the preferred method for dry materials.
  13. Fertilizer spreader run off road by another car dumped load in road and in this homeowners yard not reported but did get cleaned up by hand dead grass now but it should come back
  14. Anytime something spilled in someone’s yard - make sure reported and cleaned up immediately After they see things for awhile, they decide they do not like it and when we hear about it, things get really blown out of proportion.
  15. If true emergency - MDA is not set up to provide immediate on-scene response If life threatening or dangerous to community, always call 911 We can provide technical assistance if needed once the true emergency is taken care of Here, we provide direction on recovery and land application of the fertilizer materials even though fire and some fuel may have impacted a small portion of the materials.
  16. Lawncare spill - always happen in worst spot - they did not call this in or do anything about it - homeowner saw the dead spot after a couple of days and called this in. Dug up soil, pumped out catch basin, replanted grass - regulatory action because they did not attempt to take care of and report - lucky there was no rain and that the catch basin wasn’t full or flowed directly to storm drains and surface water.
  17. Farmers may bring in unsafe or inappropriate equipment for hauling materials away from your facility. You may have some liability if you allow a knowingly bad transport to be filled and leave from your facility. This load shifted in the tank and the wagon broke and rolled over material lost in the middle of a highly visible and high traffic intersection on the edge of town, shortly after 5pm
  18. Some people will do anything to make it fit. This is what is left of the tank that rolled with the wagon. No liquids left in the tank Collection of standing liquids and digging up of impacted soils. Miss Dig called because of utilities (many) at the intersection - some materials left in place because of proximity to fiber optics ($) Get what we can get without making the problem worse.
