Falling Tools - Safety Alert - this could have so easily been another fatality!!
1. SAFETY ALERT
On the 10th April 2014 a CIS operator working on a plant shutdown was struck by a falling hammer
weighing 7 Lbs.
The hammer had fallen approximately 25 Meters and struck the operators helmet as he was working
directly below.
The impact resulted in a cut to the head, the operator was sent to hospital and after checks and X rays was
found to have no fractures or concussion and was released from hospital the same afternoon
The investigation that followed revealed that permits had been issued for both tasks by different issuers
and both the issuers and the operators were unaware of each other’s activities.
HOW DO YOU CONTROL MULTI LEVEL WORKING?
Undoubtedly the helmet saved the operator from a more serious injury or possible death.
The helmet issued to the operator was an MSA V Guard and was date stamped January 2013
