COMMENTS:
Yudeline worked on several projects within a team and was courteous, straightforward and
developed a good working relationship with the others on the team. She provided the team with
feedback when it was asked of her.
COMMUNICATION
Written and oral communication is clear and concise. Listens well and encourages others to make
contributions. Makes requests and accepts requests from others. Documents important information
for future reference. Uses written and verbal communication to deliver pertinent information to co-
workers (i.e., follow-up and feedback).

COMMENTS:
Yudeline demonstrated very good communication skills. She worked on several quality
improVement monitoring projects and she would always make sure to ask for feedback to ensure
she completed her task correctly.

Has learned the fundamental skills required for effective job performance. Has identified the skills
needed for success in the internship and has spent the time necessary and put forth the effort required
to work toward the mastery of these skills.

Yudeline has been committed to learning the different aspects of compliance and quality
improvement. She has demonstrated having the job knowledge and technicat abilities needed and

can be depended upon to be a self-starter, work independently when necessary and always completes
work on time.
