“Transition Services”
Theterm transition services is defined as a
coordinated set of activities for a child
with a disability that:
3. +
Isdesigned to be within a results-oriented
process, that is focused on improving the
academic and functional achievement of the
child with a disability to facilitate the child’s
movement from school to post-school
activities, including
post-secondary education
vocational education
integrated employment (including supported
employment)
continuing and adult education
adult services
independent or community participation
4. +
Isbased on the individual child’s needs,
taking into account the child’s strengths,
preferences, and interests
Each student must have included in the IEP
appropriate measurable post-secondary
goals based on age appropriate assessment
related to:
Training
Education
Employment, and where appropriate
Independent living skills
The transition services (including courses of study)
needed to assist the child in reaching those goals

What do we mean?
PersonnelPlus
Transition Services are provided to
compliment the services the high school is
already providing
and related services, community
Instruction
experiences, development of employment and
vocational evaluation
All of these activities are coordinated based on
the student’s individual needs
7. +
Transition Partnership
Working in close collaboration with families and
all related professionals who can assist
Everyone has the same goal: to prepare the
student for the adult world of community life
and work.
8. +
Collaborative Service
Why bother with PersonnelPlus?
Most students need extra assistance to help them
transition to meaningful adult roles
Plus, most will need assistance to maintain those roles
No one service provider can do it all
Together we create efficient service delivery
We are optimizing all our resources, and providing
students with a wide variety of options
Collaborating helps expand what you can do
Together we can ultimately create better outcomes for
the student
Collaboration facilitates achievement of transition
goals
9. +
Entitlement versus Eligibility
A student is ENTITLED to services from school
for as long as he/she is in school
A student must be found ELIGIBLE for
services after high school
PersonnelPlus can help answer questions
about:
Where to go
Application processes and appointments
How to appeal adverse decisions
13. +
Strategies
for Successful Transition
Start early! Encourage students to apply for County Board
Services as soon as possible in order to be placed on the
waiting list.
The student should be driving the goals, wants, needs,
and actively participate in the entire process
Help the student and the teams have an awareness and
understanding of strengths and limitations
Help parents buy into the outcome of the plan and the
student becoming employed
Attend special events (transition fairs, job fairs, summer
employment, etc.)
Good collaboration with all persons and community
organizations who are involved with the student
