Japan Airlines Flight 123 was a domestic flight from Tokyo International Airport (Haneda) to Osaka International
Airport (Itami) on August 12, 1985. The Boeing 747 suffered mechanical failures 12 minutes into the flight and 32
minutes later crashed into two ridges of Mountain 100 kilometers from Tokyo, on Monday 12 August 1985.
Problem
• Tail Fin of the Airplane explode and missing in flight
• Hyraulic failure
• Unpressurrize
• Pilot get Hypoxia giving wrong command
• A/c Bulkhead not properly maintain
• Maintain 30 minutes flying before crash
• All 15 crew members and 505 out of 509 passengers died, resulting
in a total of 520 deaths and 4 survivor
CHRONOLOGY OF THE DRAMATIC
DISASTER
• Take-off from Haneda airport
• Explosive decompression occur ruptured the lines of all four hydraulic systems
• Tail fin missing – a/c control surface disabled – become uncontrollable
• Pilot key in “7700” emergency code 2 step before “Pan Pan Pan”and
“Mayday”
• Decide to turning back infact they never make it to Haneda.
• Pilot didn’t reply or answer to ATC --seems paralyzed—get “HYPOXIA”
• ATC didn’t know a/c lost all 4 hyraulics
• a/c uncontrolable—plunging up and down
• Crash over mountain after 22 minutes in flight
CAUSES & SOLUTION
• Investigation by NTSB -maintenance history– a/c got incident on bulkhead before -
landing(tailstrike)
• subsequent repair of bulkhead – not Boeing standardize –fixing- put doubler plt
• metal fatigue b/h increase- b/h break up- hyraulic lost-uncontrol
• Finding black box fdr,cvr-NTSB –sructure failure& human error causes the crash of
Japan Airlines 123
• Later on NTSB create software- emergency –structure failure-engine learn the
system and – try to correct itelf -take cover the aircraft control.
