Maintain accurate, detailed report and records, Monitor, record and report symptoms and changes in patient’s conditions, records patient’s medical information and vital signs.  Implement the nursing care through monitoring and interpreting vital signs; provision of supportive measures like physical and psychosocial needs dietary regimen, comfort, hygiene, safety, health teaching; maintenance of therapeutic environment and carrying out doctor’s order.  Formulate the nursing care plan through prioritization of health needs.  Evaluate the care given by the nurses and make necessary revisions if necessary through appropriate documentation of information relevant to patient care.  Consult and coordinate with health care team to assess, plan, implement and evaluate patient’s care plan.  Recheck contraptions of patient form time to time (Tractions, Splints, cast, IV, etc.)  Check monitoring of patient and the proper documentations (Intake and output, turnings, vital signs, etc.)  Direct and supervise the less skilled nursing, health care personnel as well as the students while supervising a particular unit in one shift.  Re-check Doctor’s orders of the shift.  Prepare patients for, and assist with, examinations, treatments or surgical procedure.  Supervise pre-operation medications  Receive endorsement and new of post-operation patient.  Heath teaching and follow-ups prior to discharge.  Proper endorsements to the next shift.
