Meet in the Middle Permission Form

This year-long project is grant-awarded from the US Department of Education to
Special Olympics on a national level. It is designed for school students, both
typically-developing, as well as those with intellectual disabilities to partner and
participate in activities which focus on interactive, inclusive and FUN
opportunities that will increase acceptance and awareness of each student’s
uniqueness, while supporting fitness, disability awareness, academic achievement,
and sports-related training. It is designed to be a win-win situation for everyone
involved.

“Meet in the Middle” will take place from September/October, 2009 until May,
2010. The projects will be individually selected by involved school student
participants, from a special ‘menu’ designed for the project. The project activities
have been assigned a level based on involvement and depth and are indicated in
the “Meet in the Middle” student folders/activity menu. This will help in
determining ‘high performers’ throughout the year. Rewards will be given at the
end of the year for students who participated based on the type and level of the
activities selected. Projects will be uploaded online at www.slideshare.com, and
will be kept private for only Special Olympics Texas viewing.

By signing this form, you are giving your son or daughter permission to participate
in this program from September/October, 2009 to May, 2010, and understand that
as part of the discovery process, diagnosis and general information about your
child’s disability may be discussed to promote understanding and awareness. As
part of the event, your child’s picture may be taken and used in promotional
materials by the school. By signing this form, you are giving your permission for
their likeness to be used.

Meet In The Middle Permission Form Revised

  • 1. Meet in the Middle Permission Form This year-long project is grant-awarded from the US Department of Education to Special Olympics on a national level. It is designed for school students, both typically-developing, as well as those with intellectual disabilities to partner and participate in activities which focus on interactive, inclusive and FUN opportunities that will increase acceptance and awareness of each student’s uniqueness, while supporting fitness, disability awareness, academic achievement, and sports-related training. It is designed to be a win-win situation for everyone involved. “Meet in the Middle” will take place from September/October, 2009 until May, 2010. The projects will be individually selected by involved school student participants, from a special ‘menu’ designed for the project. The project activities have been assigned a level based on involvement and depth and are indicated in the “Meet in the Middle” student folders/activity menu. This will help in determining ‘high performers’ throughout the year. Rewards will be given at the end of the year for students who participated based on the type and level of the activities selected. Projects will be uploaded online at www.slideshare.com, and will be kept private for only Special Olympics Texas viewing. By signing this form, you are giving your son or daughter permission to participate in this program from September/October, 2009 to May, 2010, and understand that as part of the discovery process, diagnosis and general information about your child’s disability may be discussed to promote understanding and awareness. As part of the event, your child’s picture may be taken and used in promotional materials by the school. By signing this form, you are giving your permission for their likeness to be used.
