INTRODUCTION


             Nursing is a vital aspect of health care and needs to be properly
organized. It is widely recognized that of all the resources needed for
running an organization. Management is a specialized activity required for
the running of all those social institutions which are composed of human
being. Personnel management is the part of management which is concerned
with increasing the effectiveness of human performance in any organization.
Nursing personnel management constitutes the care of clients and other
functions and goes to the heart of all health services. Human resource is the
most significant and valuable. Health manpower management implies that
health personnel are utilized in a manner which is corresponds to their skills
and competence and under conditions that promote effective work.
DEFINITIONS
Personnel management is a code of the ways of organizing and treating
individuals at work so that they will get the greatest possible realization of
their intrinsic abilities thus attaining maximum efficiency for themselves and
their group and there by giving to the enterprise of which they are part of its
determining competitive advantages and its optimum results.

mould human resources into an effective organization provides opportunity
for maximum individual contribution, under desirable working conditions,
promotes individual development and encourages mutual confidence and
understanding between employees and employers as well as among

Personnel management is a responsibility of all those who manage people as
well as being a description of the work of those who are employed as
specialists. It is that part of management which is concerned with people at
work and with their relationship with in an enterprise. It applies not only to
industry and commerce but to all fields of employment.

objective is the maintenance of those relationship on the effective working
of the undertaking.

AIM OF NURSING PERSONNEL MANAGEMENT:
             Nursing service administration is a complex of elements in
interaction. It results in output of clients whose health is unavoidably
deteriorating, maintained or improves through input of personnel and
material resources used in an orderly process of nursing services.
   1. to employ those nurses best fitted with education, skills and
      experience to perform the prescribed work.
   2. to attract and attain competent professional nurses and other personnel
      in order that nursing service may be effectively and economically
      achieved and constantly improved.
   3. to provide services which contribute to health, development of
      efficient and contentment of the personnel.
OBJECTIVES OF NURSING PERSONNEL MANAGEMENT
               The healthmanpower which has been planned and planned
  and produced accordingly must also be properly utilized for maximal
  effectiveness of the health services.
     • To guarantee fairness in the maintenance in the maintenance of the
         discipline.
     • To up grade and promote the existing staff where ever possible.
     •   To take all practical steps to avoid excessive hours of work.
     •   To ensure the greatest practicable degree of permanent and
         continuous employment.
     • To maintain standards of remuneration comparable to those
         prevailing in similar position elsewhere.
     • To provide and maintain a high level of physical working
         conditions with particular reference to prevention of accidents,
         maintenance of health and rehabilitation of sick and injured
         employee.
     • To maintain effective methods of regular consultation between the
         administration and the nurses.
     •   To provide maximum opportunities to nurses for individual
         development to enable them to duties and make them fit for
         promotions.
     •   To encourage social and recreational facilities for employees.
     • To develop appropriate scheme for welfare of employees.
     • To maintain personal records.
ELEMENTS OF PERSONNEL MANAGEMENT
         The main elements of personnel management are as follows-
 Recruiting employees, who are suitable, qualified and has the
         requisite skills, attitude and experience for the job and fulfilling the
         objectives of the hospital.
      Retaining such employees, ensuring that each employee is
         provided with a satisfactory work environment and favourable
         service conditions and gets job satisfaction.
      Maintaining good interpersonal and employee relationshipthrough

         a good communication system.
      Ensuring that the employee are aware of subscribe to and follow

         the objective of the hospital, dealing with patients with competence
         and compassion and believing in the dignity of the person under
         their care records training discipline.

SJob Summary
Nursing Superintendent is responsible to the Medical Superintendent, in a
hospital having 200 or above bed strength. She is accountable for the safe
and efficient running of the various nursing departments in the hospital. She
is assisted in carrying out her duties, by the Deputy Nursing Superintendent/
Assistant Nursing Superintendent, ward supervisors and clerical, linen room
and domestic staff.
Nursing Services
     1. Participates in the formulation of the philosophy of the hospital in
         general and those specific to the nursing service.
     2. Determines goals, aims, objectives and policies of the nursing
         services.
     3. Implements hospital policies and rules through various nursing
         units.
     4. Decides and recommends personnel and material requirement for
         running various nursing services departments of the hospital.

2. NURSING SUPERINTENDENT GR II

Standard Norms
Since it is the second level nursing supervisory role, it needs at lest the
Nursing Superintendent Gr II for three senior staff nurses (1:3).
Job Summary
She/he is responsible for developing and supervising nursing services of a
department or a floor consisting of two or more wards or units managed by
the senior staff nurses. These units may be inpatient wards, out-patient
department clinics, operation theatres, obstetric units, central supply
department etc. she/he is responsible to the Nursing superintendent Gr I.


Patient Care and Ward/Unit Management
     1. Organises and plans nursing care activities of the department of
         floor according to the hospital policies and service needs.
     2. Plans staffing pattern and the other necessary requirements of
         her/his department.
     3. Complies and submits nursing statistics to the concerned
         authorities.
     4. Conducts and attends to the departmental and interdepartmental
         meetings/conferences from time to time.
     5. Makes regular rounds of her/his department.
     6. Ensures to the safety and general dealings of the department.
     7. Looks into general comforts of the patient and his/her relatives.
     8. Receives report from the Night Supervisor of her/his departments.
     9. Evaluates nature and quantum of care required in each unit/ward.
     10. Makes rotation plan for the nursing staff and domestic staff under
         her/his jurisdiction.
     11. Plans ward management with the each ward/unit.
     12. Reinforces the principles of good ward management in ward.
     13. Helps ward/unit supervisors to procure their ward/unit.
14. Supervises the proper use and care of the equipment and supplies in
          the department.
    15. Acts as the public relation officer of the unit and deals with the
          problems faced by the ward supervisor if any, specially with Group
          “D” employees, patient attenders.
    16. Keeps the nursing superintendent Gr I and office informed of the
          needs of the nursing units/wards under her/his charge and of any
          special problem/problems.
    17. Officiates in the absence of Nursing Superintendent Gr I.


Educational Function
    1. Arranges classes and clinical teaching of nursing students in the
          departments, related to the specially experience.
    2. Implements the ward teaching programme and clinical experience
          of the students with the help of doctors and nurses.
    3. Does counseling and guidance of staff and students.
    4. Arranges and conducts staff development programmes of her/his
          department.
    5. Assists in planning for and participation in the training of auxiliary
          personnel.


General
    1. Escorts, Nursing Superintendent Gr I, Medical Superintendent, and
          special visitors for hospital rounds.
    2. Arranges and participates no professional and social function of
          staff and students.
3. Acts as a Liaison officer between the nursing department and
     higher hospital authorities.
 4. Carries out any other duties delegated by the Nursing
     Superintendent Gr I.


3. ASSISTANT NURSING SUPERINTENDENT
            She is responsible for developing and supervising nursing
services of a depertment of a floor consisting of two or more wards or
units managed by the ward supervisors. These units may be in patient
ward, out patient departments / clinics, operation theater, obstetrics,
central supply department etc. she is responsible to the nursing
superintendent and deputy nursing superintendent.
1.patient care and ward management-
    Organize and plan nursing activities of the department of floor
      according to the hospital policies and services.
    Plans staffing pattern and other necessary requirements of her
      department.
    Compiles and submit nursing statistics to the concern autrorities.
    Conduct and attend to departmental and inter departmental meeting

    Seeks to the safety and general cleanliness of the department.
    Looks into general comfort of the patient.
    Receives report from the night superintendent.
    Plans ward management.
    Reinforces the principal of good ward management.

    Act as public relations officer.

4. DEPUTY NURSING SUPERINTENDENT
Job summary-
Responsible for nursing superintendent and assists her in the nursing
service administration of the hospital. She is independently in charge of
nursing service department.
1.nursing service-
   • Officiates in the absence of the nursing superintendent.
   • Participates in the recruitment of nursing services, philosophies,
      objectives, and policies.
• Assist in the recruitment of the staff and students.
   • Makes master duty roste of the nursing staff.
   • Helps in allocation of nursing personnel to various nursing services
      departments.
   • Keeps records and reports of the nursing services.
   • Assist in planning and organizing the new units.
   • Maintains confedential reports.
   • Interpret the policies and procedures of the hospital.
   • Acts as a liason between the nursing superintendent and the
      nursing staff.
   • Receives night reports from the night supervisors.
2. Educational functions-
   • Does counseling guidance to the staff and students.

   • 3.General duties-

5. CHIEF NURSING OFFICER
   Job summary
                   The chief nursing officer will have the role of co-ordination
   and overall supervision and guidance. She is responsible to the director of
   the institution. She is accountable for the staff and efficient running of
   the nursing services in the main hospital and the centers of the
   institutions.
   Nursing services-
       Participates in the formulation of the philosophy of the institutions
          in general and those specific to the nursing service.
       Determine goals, aims, objectives, policies of the nursing services.
       Implement institutions policies and rules through hospital.
       Interview and recruits nursing staff.
       Ensure safe and efficient nursing care rendered to the patients of
          the hospital.
       Ensure standards of care is maintained.
       Ensure the welfare of the patients their relatives.
       Gives counseling and guidance to the sub-ordinates.
       Maintains discipline among nursing staff.
       Participates in the hospital meetings and conferences.


6. SENIOR STAFF NURSE/ WARD SISTER/ NURSING SISTER
Educational Qualifications
1. General : Pre-University course/10+2 or equivalent exam.
2. Professional : 3 years General Nursing/9 months/6 months Midwifery            /
Psychiatric Nursing Diploma certificate, recognized by Indian Nursing
Council

Standard Norms
Since it is a first level nursing supervisory role which needs at least one
senior staff nurse for five staff nurses. (1:5).
Job summary
Senior staff nurse is a first level nursing supervisor who is accountable for
nursing care management of a ward or a unit assigned to her/him. She/he is
responsible to the Nursing Superintendent Gr II for her/his ward/unit
management. She/he takes full charge of the ward and assigns work for
various categories of nursing and non-nursing personnel working with
her/him. She/he is responsible for safety and comforts of the patients in
her/his ward.


Direct Patient Care
     1. Ensures proper admission, discharge of patients.
2.   Plans nursing care and makes patient assignments as per their
         nursing needs.
    3. Assists in the direct care/provides direct care to patients as and
         when required.
    4.   She/he sees that total health needs of his/her patients are met.
    5. Ensures safety, comfort and good personal hygiene of her/his
         patient.
    6. Ensures safety, comfort and good personal hygiene of her/his
         patient.
    7. Ensures that helpless patients are regularly fed as per direction.
    8. Ensures that proper observation records of the patients are made
         and necessary information imparted to the concerned authorities.
    9. Takes nursing rounds with staff and students.
    10. Makes rounds with doctors, assisting them in diagnosis and
         treatment of patients.
    11. Implements doctors instructions concerning patient treatment.
    12. Assists patient and his/her relatives to adjust in the hospital and its
         routine.
    13. Co-ordinates patient care with other departments.


Supervision and Administration
    1. Ensures safe and clean environment for the ward/unit/special
         department.
    2. Makes duty and work assignments.
    3. Indenting and procurement of ward supplies and equipments and
         keeps records.
    4. Does regular inventory checking of her/his ward/unit.
5. Makes list for condemnation of articles and submits it to all the
       concerned.
    6. Assists in making ward requirement.
    7. Establishes and reinforces standards prescribed in the procedures
       and manuals of hospital and policies that are in force.
    8. Acts as a liaison officer between ward staff and hospital
       administration.
    9. Maintains good public relations in her/his ward/unit.
    10. Ensures that ward statistics are regularly submitted.
    11. Maintains discipline among the personnel working in the particular
       ward/unit, e.g., staff nurses, students and domestic staff.
    12. Deals appropriately with any adverse situation that occurs in the
       ward/unit and report to the concerned authorities.
    13. Reports about any medicolegal case in the ward/unit.
    14. ensures that students get desired learning experience in the
       ward/unit.
Educational Function
    1. Organises orientation programmes for new staff
    2. Organiss formal and informal ward teaching, conducts bedside
       clinics and demonstration etc.,
    3. Conducts ward conferences/meetings.
    4. Gives incidental teaching to patient, relatives, staff nurses, students
       and the domestic staff.
    5. Guides in formulation of nursing care studies, and nursing care
       plans etc.
    6. Evaluates the students performances and submit reports to the
       school authorities.
7. Helps in medical and nursing research.
     8. Encourages staff development programme in her/his ward/unit.


7. STAFF NURSE
Educational Qualifications
   a) General: Pre-University course/10+2 or equivalent exam.
   b) Professional: 3 years General Nursing/9 months/6 months
       Midwifery /Psychiatric Nursing Diploma certificate, recognized by
       Indian Nursing Council

Standard Norms and INC (Nurse: Patient Ratio)
1. General Wards
a. 1:3 (Hospital attached with school or college or nursing).
b. 1.5 (Hospital not attached with school or college of nursing).
2. ICU, ICCU and other specialty       1:1 for 24 hours.
3. Labour room                        4 in each shift
4. Operation theatre                   3 for 24 hours per table.
5. Out-patient department              1 in each clinic room of the OPD.
6. Casualty and emergency              1:1 in each shift
7. Pediatric Unit                      1:2 beds
8. And 30 per cent leave reserve post of staff nurses should be maintained.




Job summary
Staff nurse is a first level professional nurse who provides direct patient care
to one patient or group of patients assigned to her/him during duty shift and
assist in management of wards/units/special departments. She/he is directly
responsible to Senior Staff Nurse or ward in change Nurse/Nursing
Superintendent Gr II.


Duties and Responsibilities
Direct Patient Care
  1. Carry out the procedures of admission and discharge of the patient
  2. Makes beds of serious patients and helps or guides students or Group
      “D” employees to make beds, by supplying linen.
  3. Maintains personal hygiene and comforts of the patient.
  4. Attends to the nutritional needs of the patient and feeds the helpless
      patients.
  5. Maintains clean and safe environment for the patient.
  6. Implements and maintains ward policies and routines.
  7. Co-ordinates patient care with other team members.
  8. Take rounds with the doctors when called to list new orders and see
      that they are carried out.
  9. performs various technical tasks related to nursing care.
          a. Administration of medication, i.e., tablets, injections, infusions
             and transfusion on prescription, or according to standing
             instructions.
b. Assisting doctors in various medical and surgical diagnostic
           procedures by preparing patients and getting ready with
           required things.
       c. Performing simple diagnostic procedures viz. urinalysis
           hemoglobin % etc.,
       d. Collecting and sending of specimens for laboratory diagnostic
           procedures.
       e. Recording of vital signs, i.e., temperature, pulse, respiration
           and blood pressure.
       f. Performing gastric lavage, giving enema etc.,
       g. Prepares patients for operations and see that he/she is sent to
           operation theatre with all necessary papers and medications.
       h. Takes care of eyes, ears, back, bowel, bladder, perineum, and
           breast etc., whenever needed.
       i. Observes all patient conditions and take suitable action
           accordingly and/or reports changes toward in charge and/or the
           doctor.
       j. Give expert bed-side nursing to all patients.
       k. Attends last offices in case of a patient dying during shifts and
           arrange to preserve dead body in mortuary, or hand over the
           body      with        respect    to      concerned       family
           members/relatives/authorities.


Ward/Unit Management
   1. Helps the ward in charge to carry out her/his work or act as ward in
      change during their absence.
2. Maintains general cleanliness of the ward and the sanitary
       annexure.
    3. Supervises the duties of Group “D” employees and guides them and
       reports accordingly.
    4. Writes the diet register and supervises the distribution of diet and
       report if any, necessary.
    5. Maintains scheduled poisonous drug registers.
    6. Supervises nursing care and other tasks carried out by the students.
    7. Maintains duty room trays, sterilizes instruments and see that
       procedural trays are in readiness.
    8. Take over from duty nurse of the previous new and serious patients,
       instruments, supplies, drugs etc and handover the same accordingly.
    9. Maintains all the records pertaining to ward/unit.
        a. Maintains case papers, investigation reports etc.
        b. Maintains vital signs charts, intake-output charts and other
           special charts if necessary.
        c. Takes special care of medicolegal case papers and records.
        d. Writes day and night orders and maintains ward statistics.


Operation Theatre Management
    1. Maintains aseptic environment of the operation theatre.
    2. Autoclaving of articles, instruments, gloves, linen etc. required for
        various types of surgery.
    3. Receives patients from the ward intact for surgery.
    4. Prepares anesthetic trolley and trolley for surgery, according to
        type and procedures.
5. Assist the surgeon and anesthetist in every step, skillfully while
          performing various types of surgery.
     6.   Indenting and procuring surgical instruments, drugs, gloves,
          suturing materials and O2, N2, CO2 etc. required for operation
          theatre.
     7. Maintains records and reports pertaining to the operation theatre.
     8. Maintains safety of the Boyle’s apparatus, oxygen cylinder,
          nitrogen cylinder, anesthetic drugs, and autoclave etc. in the
          operation theatre.
Management of Labour Room
    1. Preparation of expectant mother for aseptic safe delivery.
    2. Conducts normal deliveries and reports.
    3. Attends and assists the doctors in all obstetrical emergencies.
    4. Attends and assists in difficult and abnormal deliveries.
    5. Takes care of the newborn and premature babies.
    6. Maintains records and reports pertaining to labour room.
    7. Indent and procure necessary drugs, supplies linen etc. to labour
          room.


Management of Post operative/ICU/Burns units
    1. Indent all procurement of all the necessary equipments, drugs, O2
          cylinder, which are required for the units.
    2. Operates      ECG,      EEG,   cardiac resuscitation   etc.   or   other
          sophisticated high-tech machines whenever needed or assist the
          doctors in operating such machines.
Psychiatric Unit
    1. Assists the doctors in admission and discharge of patients.
2. Prepares patient for ECT and other procedures, and therapies.
    3. Assists in management of aggressive, suicidal as grief as other
       symptoms of the patients.
    4. Maintains records and reports of the units.


Educational Function
    1. Helps in orientation of new staff and students.
    2. Teaches and guides the domestic staff (Group “D”) for handling
       bedpans, urinals, etc.
    3. Carries out health leaching for individual or group of patients.
    4. Extends co-operation and participates in clinical teaching.
    5. Provides for and demonstrates methods and procedure whenever
       needed.
    6. Participates in in-service education programme.

school she/he will be responsible for the smooth implementation of the INC
syllabus and school administration.




Duties and Recommendation

     2. Scrutinising applications, preparing merit lit and calling them for
        interview.
     3. Conducting interview through the set-up committee and selecting
        candidates and informing them.

        along with teaching materials connected with that, e.g., nursing
        arts, anatomy, physiology, etc.

            Bhore committee recommended posting of PHN at the primary
health centers but unfortunately most states have failed to do so. The only
posting of PHN is at the district level is DPHN.
    To help in organization of maternaland child health programmes as a
      whole and in the implementation of the special plan scheme, centrally
      sponcered and othervise in particular like the immunization
      programme, training of traditional birth attendants and their active
      involvement in MCH, FP etc.
 To promote health and nutrition educative activities through the lady
   health visitors, ANMs by providing them with taking points and
   printing materials produced by various agencies.
 To ensure that the lady health visitors / MHW female multi purpose
   workers etc integrated MCH FP and health nutrition education in their
   day to day activities.
 To help in developing school health porgrammes in the district.
 To ensure regular supply of equipment records register drugs vaccines
   and other sundries necessary for MCH work in the primary health
   centers and sub centers by assisting store keeper in procuring and
   distributing the supplies.
 Ensure the maintenance of prescribed records.
 To help the statistical officer in the district family welfare bureau in
   compiling the periodical progress reports of MCH activities.
 To review the periodical progress reports of MCH activities FP work
   done by the LHV/ MPHW.
 To give technical guidance supervision and support to the female
   MPHW LHV PHN working in the PHC.
 To investigate into complaints against female medical personnel in the
   district and submit reports to the district M.O.
 To provide continuining education of the female MCH / FP in the
   district through in service education sources.
 To work together with the functioneres of other govt dept like social
   welfare, rural development, education engaged in programmes for
   women and children.
 To co oprate MCH / FP activities undertaken through the voluntary
      organization in the district and provide health inputs to the possible
      extent for mothers and children.
    To tour for minimum 15 days in a month and visit primary health

      centers, subcenters, villages, dais, balwadi, mahila mandals etc
      according to an advance programmes duly approves by the DMO.
    Attending to problems reffered to her.
    Provides health services to the community according to its policies of
      the department in the area of preventive, promotive, curative and
      rehabilitative care.


2. COMMUNITY HEALTH OFFICER


      One community health officer will be posted at each new primary
health center and will cover 30,000 population. She will be under direct
administrative and technical control of MO-incharge of PHC.




DUTIES AND RESPONSIBILITIES-
Control of communicable diseases-
   o Ensure that all necessary steps are being taken for the control of the
      communicable diseases in the village.
   o She should report any outbreak of an epidemic to Medical Office in-
      charge.
   o Assist the Medical Officer of the primary health centre in taking the
      necessary action in case of any epidemic in the PHC area.
Maternal and child health-
o Supervise and guide the health assistant and health workers and
     actively involve the health guides and trained dais in the effective
     implementation of the programme for maternal and child health.
School health-
  o Visit schools in the phc area at regular interval and arrange for
     medical checkups by MO PHC immunization environmental
     sanitation and health education.
  o Make arrangements for the treatment and follow-up of those students
     found to have defects.


Universal immunization programme-
  o Supervise and guide the health assistants and health workers and
     actively involve the health guides and trained dais the effective
     implementation of the expanded programme of immunization.
Family planning-
  o Supervise and guide the health assistants and health workers and
     actively involve the health guides and trained dais the effective
     implementation of the family planning programme.
Nutrition
  o Supervise and guide the health assistant and workers and actively
     involve     the   health   guides   and   trained   dais   the   effective
     implementation of the nutritional programme such as administration
     of vitamin A and iron and folic acid tablets.
Environmental sanitation-
o Help to ensure that all the steps are being taken to provide safe
     drinking water and for the improvement of environmental sanitation
     in the village.
Treatment for minor disorders-
  o Provide guidance to the health assistant health workers and primary
     school teachers in the treatment of minor disorders.
Community involvement and health education-
  o Participate in the village committee or village panchayat meetings to
     assess the health needs of the community
  o To discuss the health programmes with the community and to enlist
     their co-operation in these programmes.
  o Maintain close liason between the Block development officer and her
     staff.
  o Work closely with the community leaders and community
     organizations.
  o Organize camps, meetings, health educations, demonstrations,displays
     of posters,exhibitions,films and involve all health care workers.
Training of health personnel and community level workers-
  o Educate community about health guide schemes
  o Organize and conduct the training programme for health guides,
     teachers, dais.
  o Be primarily responsible for the continuing education of health
     guides, teachers.
  o Assist the M.O PHC in staff development programmes
  o Be actively involved in the field training components of training
     programmes conducted by the health and family welfare.
Management and supervision-
   o Assist the M.O, PHC in conducting field investigations
   o Ensure that the subcenters are properly maintained and managed by
      the health workers and health assistants.
   o Ensure about supplies and equipment to PHC
   o Ensure the regular replenishment of kits and supplies.
   o Scrutinize the work plans of health assistanta and health workers.
   o Supervise and maintain prescribed records.
   o Organize meetings at regural intervals.
   o Keep notes of all her activities in the PHC.
   o Any other duty assigned by the M.O should be carried out by the
      CHO.


3. LADY HEALTH VISITOR

      Health visitors are registered nurses who have undertaken further
training to work as part of a primary health care team. As their name
suggests, their role is to promote mental, physical and social well-being in
the community by giving advice and support to families in all age groups.
Limited resources and staff within the NHS have traditionally meant that
their work has been focused on childhood development, but the scope to
expand their roles is slowly improving. The Healthy Child Programme
published in October 2009 influences the service available to families. It is
presented in three key documents: The First Five Years, The Two Year
Review and The Healthy Child Programme for 5-19 year olds.
They usually work with mothers once postpartum care is handed over from
the midwives, advise on feeding, care and support to both infants and
parents, provide routine child development checks and have responsibility
for child protection issues. They are also able to help people of any age who
suffer from chronic illness or live with a disability. They may run health
promotion schemes such as stop-smoking clinics.

AREA OF WORK-

Clinics, fields, maternity homes, and hospitals, family planning camps,
emergency duties in flood and epidemic areas.




Clinics-

Organizes antenatal clinics and postnatal clinics, well baby and family
planning clinic,

 runs full time clinic in the absence of the doctor supervises and guides
ANMs dais FP etc principal motivator for achieving net family planning
targets independently rns 20 beds or more maternity homes assists doctors in
the insertion of iud and other emergencies responsible for error of audit,
condemnation.

In the ward-

Managers maternity ward in the maternity home which are 20 beded or more
bedded. Her duties are similar to the duties of a ward sister in any other
hospital, concerning patient care, taking charge of the ward stock,
maintaining the cleanliness and safety of the ward.

Field duties-

   o Provides health care to all individuals families and community in any
      field that she is appointed.
   o Trains supervises and guides the health workers in the delivery of
      health care services to the community.
   o Plans health and family welfare programmes along with health
      workers.
   o Carries coordinate the work of male and female health workers and
      other personnel.
   o Arranges educational programmes for the community.
   o Assists in identifying areas requiring study research approved by the
      community health department.
   o Assists in conducting medical examinations.

4. AUXILIARY NURSE MIDWIFE (ANM)

             The roles of Auxiliary Nurse Midwife (ANM) and ASHA have
been integrated in various ways. The ANM will hold weekly/fortnightly
meeting with ASHA, and provide on-job raining by discussing the activities
undertaken during the week/fortnight and provide guidance in case ASHA
encounters any problem. ANMs will also act as resource persons for the
initial and periodic training and also ensure that during the training ASHA
gets the compensation for performance and also TA/DA for attending the
training schedule.
She will also guide ASHA in bringing the beneficiary to the outreach
session. She will utilize ASHA in motivating the pregnant women for
coming to the Sub-Centre for initial check-ups and also take ASHA’s help in
bringing married couples to sub centres and motivating pregnant women for
taking full course of Iron and Folic Acid (IFA).

   o She assists in conducting MCH and FP clinics
   o Surveying the area and maintaining contact with local dais.
   o Supervision of work of local dais.
   o Regular home visits.
   o Antenatal intranatal and postnatal care.
   o Maintaining records of daily work.
   o Assisting in registration of birth and death.
   o Providing medical care aspects.
   o Does any other duties assigned by the PHN.

                       NURSES AT UNION LEVEL

1. NURSING OFFICER-

             At the central level there is a post of nursing adviser in the
medical division of directorate general of health services. The advisor is
directly responsible to the deputy director general. The nursing advisor is
assisted-by nursing and support staff for all her work.

    She advices the DGHS ministry health family welfare as well as other
      ministers, departments.
 The nursing advisor also takes care of administrative aspects of Raj

   Kumari Amrit Kaur college of nursing and lady hardinge health
   school both in delhi.
 Nursing advisor being the head of the nursing services in the country
   responsible for advising the govt of India on all matters concerning
   nursing service nursing education and specialization in nursing.
 Guides supervises and co ordinates nursing services and nursing
   education in the states.
 Plans total nursing manpower requirements of the country in the area

   of nursing service, nursing education and continuning education etc in
   co operation and co ordination with state govt units and voluntary
   agencies.
 Responcible for developing the national and international projects
   concerning nursing in India.
 She is programme officer for nursing section in the office of the
   DGHS which involves components of all the national health
   programmes.

 2.DEPUTY NURSING ADVISOR-

       Training of various categories of nursing personnel in family
         on an all India basis
       periodical assessment of the needs nursing personnel working
         in terms of quality
       advising state the situation
       advising the government of India in terms of policy making as
         reguard nursing training
3. NURSING OFFICER

    Assists nursing advisor in all matters of nursing
    Relieving nursing advisor as and when required
    International assistance UNICEF equipment             and supplies and
      vehicles for all training institutions of nurses / ANM.
    Organize and conduct refresher courses on and all India basis
    Representing on committees gives advice and teaching as and when
      required specially in the area of public health nursing.

             NURSING PERSONNEL AT STATE LEVEL

                           RESEARCH STUDIES

JOB     SATISFACTION          AND        TURNOVER      AMONG        NURSES:
INTEGRATING RESEARCH FINDINGS ACROSS STUDIES

Abstract

A meta-analytic study investigated the causal relationships among job
satisfaction, behavioral intentions, and nurse turnover behavior. A
theoretical model was proposed in which behavioral intentions were viewed
as a direct antecedent to turnover behavior. Job satisfaction was expected to
be indirectly related to turnover by virtue of the mediating role of behavioral
intentions. Consistent with these expectations, a strong positive relationship
was indicated between behavioral intentions and turnover; a strong negative
relationship between job satisfaction and behavioral intentions; and a small
negative relationship between job satisfaction and turnover. The results of
the modifier analysis suggested that effect sizes are fairly robust to
differences in study designs, response rates, and methods of measuring job
satisfaction, but the manner in which behavioral intentions were
operationalized appeared to moderate the relationship between behavioral
intentions and turnover and job satisfaction. Of variables related to nursing
job satisfaction, work content and work environment had a stronger
relationship with job satisfaction than economic or individual difference
variables.



THE DELIVERY OF QUALITY NURSING CARE: A GROUNDED
THEORY STUDY OF THE NURSE’S PERSPECTIVE


This grounded theory study explored and described the delivery of quality
nursing care from the perspective of nurses. Data were gathered by tape
recorded interviews, published literature and some participant observation.
Ten registered nurses from an acute-care public hospital located in Perth,
Western Australia were interviewed. Twelve additional transcripts from
interviews conducted by postgraduate students were also used. Quality
nursing care was perceived to relate to the degree to which patients’
physical, psychosocial, and extra care needs were met. The consequences of
quality care were interpreted as ‘therapeutic effectiveness’, where the
therapy provided by nurses was perceived to positively affect patients’
healing. This was gauged by the patient’s psychosocial and physical
response to illness, safety, and satisfaction. Therapeutic effectiveness was
facilitated by the development of positive relationships between nurses and
patients, nurse’s positive attributes and competent practices, as well as a
functional nursing team. The problem of nurses’ inability to consistently
provide quality nursing care to all patients was identified. Insufficient time
(caused by a lack of human and physical resources) was perceived as the
main reason for this. Dissatisfaction and stress in nurses was related to this
problem. To deal with this, nurses used a process named ‘selective
focusing’. Work was planned to most effectively utilize the time available,
within the parameters of safety. Implications for nursing practice, education
and management are discussed, and directions for further research are
provided.


NURSE-STAFFING LEVELS AND THE QUALITY OF CARE IN
HOSPITALS

      Background       It is uncertain whether lower levels of staffing by
      nurses at hospitals are associated with an increased risk that patients
      will have complications or die.
      Methods             We used administrative data from 1997 for 799
      hospitals in 11 states (covering 5,075,969 discharges of medical
      patients and 1,104,659 discharges of surgical patients) to examine the
relation between the amount of care provided by nurses at the hospital
and patients' outcomes. We conducted regression analyses in which
we controlled for patients' risk of adverse outcomes, differences in the
nursing care needed for each hospital's patients, and other variables.
Results        The mean number of hours of nursing care per patient-
day was 11.4, of which 7.8 hours were provided by registered nurses,
1.2 hours by licensed practical nurses, and 2.4 hours by nurses' aides.
Among medical patients, a higher proportion of hours of care per day
provided by registered nurses and a greater absolute number of hours
of care per day provided by registered nurses were associated with a
shorter length of stay (P=0.01 and P<0.001, respectively) and lower
rates of both urinary tract infections (P<0.001 and P=0.003,
respectively) and upper gastrointestinal bleeding (P=0.03 and
P=0.007, respectively). A higher proportion of hours of care provided
by registered nurses was also associated with lower rates of
pneumonia (P=0.001), shock or cardiac arrest (P=0.007), and “failure
to rescue,” which was defined as death from pneumonia, shock or
cardiac arrest, upper gastrointestinal bleeding, sepsis, or deep venous
thrombosis (P=0.05). Among surgical patients, a higher proportion of
care provided by registered nurses was associated with lower rates of
urinary tract infections (P=0.04), and a greater number of hours of
care per day provided by registered nurses was associated with lower
rates of “failure to rescue” (P=0.008). We found no associations
between increased levels of staffing by registered nurses and the rate
of in-hospital death or between increased staffing by licensed practical
nurses or nurses' aides and the rate of adverse outcomes.
Conclusions      A higher proportion of hours of nursing care provided
      by registered nurses and a greater number of hours of care by
      registered nurses per day are associated with better care for
      hospitalized patients




SUMMARY

      Till now we have discussed about the duties and responsibilities of
various categories of nursing personnel that includes aims of nursing
personnel management, objectives of nursing personnel, various categories
of personnel – clinical or institutional level, college level, school level and at
community level.




CONCLUSION

      Personnel management means the task of dealing human relationships
with in an organization. Aim of nursing personnel management is to employ
those nurse best fitted with education, skill and experience to perform the
prescribed work. Objectives are to guarantee fairness in the maintenance of
discipline and to upgrade and promote the existing staff wherever possible.

  • 1. INTRODUCTION Nursing is a vital aspect of health care and needs to be properly organized. It is widely recognized that of all the resources needed for running an organization. Management is a specialized activity required for the running of all those social institutions which are composed of human being. Personnel management is the part of management which is concerned with increasing the effectiveness of human performance in any organization. Nursing personnel management constitutes the care of clients and other functions and goes to the heart of all health services. Human resource is the most significant and valuable. Health manpower management implies that health personnel are utilized in a manner which is corresponds to their skills and competence and under conditions that promote effective work. DEFINITIONS Personnel management is a code of the ways of organizing and treating individuals at work so that they will get the greatest possible realization of their intrinsic abilities thus attaining maximum efficiency for themselves and their group and there by giving to the enterprise of which they are part of its determining competitive advantages and its optimum results. Thoma.G.Spates (2004) Personnel management is that activity in an enterprise whiches serves to mould human resources into an effective organization provides opportunity for maximum individual contribution, under desirable working conditions, promotes individual development and encourages mutual confidence and understanding between employees and employers as well as among employees themselves.   • 2. Personnel management is a responsibility of all those who manage people as well as being a description of the work of those who are employed as specialists. It is that part of management which is concerned with people at work and with their relationship with in an enterprise. It applies not only to industry and commerce but to all fields of employment. International institution of personnel management(1963) Personnel management is the part of management function which is primarily concerned with the human relations with in an organization. Its objective is the maintenance of those relationship on the effective working of the undertaking. Indian institute of personnel management(1962) AIM OF NURSING PERSONNEL MANAGEMENT: Nursing service administration is a complex of elements in interaction. It results in output of clients whose health is unavoidably deteriorating, maintained or improves through input of personnel and material resources used in an orderly process of nursing services. 1. to employ those nurses best fitted with education, skills and experience to perform the prescribed work. 2. to attract and attain competent professional nurses and other personnel in order that nursing service may be effectively and economically achieved and constantly improved. 3. to provide services which contribute to health, development of efficient and contentment of the personnel.

OBJECTIVES OF NURSING PERSONNEL MANAGEMENT The healthmanpower which has been planned and planned and produced accordingly must also be properly utilized for maximal effectiveness of the health services. • To guarantee fairness in the maintenance in the maintenance of the discipline. • To up grade and promote the existing staff where ever possible. • To take all practical steps to avoid excessive hours of work. • To ensure the greatest practicable degree of permanent and continuous employment. • To maintain standards of remuneration comparable to those prevailing in similar position elsewhere. • To provide and maintain a high level of physical working conditions with particular reference to prevention of accidents, maintenance of health and rehabilitation of sick and injured employee. • To maintain effective methods of regular consultation between the administration and the nurses. • To provide maximum opportunities to nurses for individual development to enable them to duties and make them fit for promotions. • To encourage social and recreational facilities for employees. • To develop appropriate scheme for welfare of employees. • To maintain personal records. ELEMENTS OF PERSONNEL MANAGEMENT The main elements of personnel management are as follows-
  • 4.  Recruiting employees, who are suitable, qualified and has the requisite skills, attitude and experience for the job and fulfilling the objectives of the hospital.  Retaining such employees, ensuring that each employee is provided with a satisfactory work environment and favourable service conditions and gets job satisfaction.  Maintaining good interpersonal and employee relationshipthrough a good communication system.  Ensuring that the employee are aware of subscribe to and follow the objective of the hospital, dealing with patients with competence and compassion and believing in the dignity of the person under their care records training discipline. VARIOUS CATEGORIES OF NURSING PERSONNEL The nursing personnel categories are mainly divided in three ways- 1. clinical area or institutional staffs 2. college of nursing staff 3. school of nursing staff 4. public health or community nursing organization staff 5.   • 6. NURSING PERSONNEL AT STATE LEVEL:  Deputy director  Assistant deputy director  Gazetted assistant. CLINICAL / INSTITUTIONAL NURSING PERSONNEL 1. NURSING SUPERINTENDENT GRADE I Educational Qualifications 1. General : Pre-University course/10+2 or equivalent exam. 2. Professional : 3 years General Nursing/9 months/6 months Midwifery / Psychiatric Nursing Diploma certificate, recognized by Indian Nursing Council or Revised General Nursing and Midwifery/Psychiatric nursing Diploma/Certificate Recognized by Indian Nursing Council. or Basic B.Sc Nursing from a Recognized University according to Indian Nursing Council norms 3. Registration : Registered with the A.P State Nursing Council/Indian Nursing Council (INC) 4. Experience : Should have experience as Nursing Superintendent Gr II. Standard Norms There should be one Nursing Superintendent Gr I for 200 bedded hospital, one Nursing Superintendent Gr I for 2-4 Nursing Superintendent Gr II.
  • 7. SJob Summary Nursing Superintendent is responsible to the Medical Superintendent, in a hospital having 200 or above bed strength. She is accountable for the safe and efficient running of the various nursing departments in the hospital. She is assisted in carrying out her duties, by the Deputy Nursing Superintendent/ Assistant Nursing Superintendent, ward supervisors and clerical, linen room and domestic staff. Nursing Services 1. Participates in the formulation of the philosophy of the hospital in general and those specific to the nursing service. 2. Determines goals, aims, objectives and policies of the nursing services. 3. Implements hospital policies and rules through various nursing units. 4. Decides and recommends personnel and material requirement for running various nursing services departments of the hospital. 5. Interviews and recruits nursing staff. 6. assists in student selection and recruitment of other auxiliary staff whose duties are related to nursing. 7. RESEARCH STUDIES JOB SATISFACTION AND TURNOVER AMONG NURSES: INTEGRATING RESEARCH FINDINGS ACROSS STUDIES Abstract A meta-analytic study investigated the causal relationships among job satisfaction, behavioral intentions, and nurse turnover behavior. A theoretical model was proposed in which behavioral intentions were viewed as a direct antecedent to turnover behavior. Job satisfaction was expected to be indirectly related to turnover by virtue of the mediating role of behavioral

Consistent with these expectations, a strong positive relationship was indicated between behavioral intentions and turnover; a strong negative relationship between job satisfaction and behavioral intentions; and a small negative relationship between job satisfaction and turnover. The results of the modifier analysis suggested that effect sizes are fairly robust to differences in study designs, response rates, and methods of measuring job satisfaction, but the manner in which behavioral intentions were operationalized appeared to moderate the relationship between behavioral intentions and turnover and job satisfaction. Of variables related to nursing job satisfaction, work content and work environment had a stronger relationship with job satisfaction than economic or individual difference variables. THE DELIVERY OF QUALITY NURSING CARE: A GROUNDED THEORY STUDY OF THE NURSE’S PERSPECTIVE This grounded theory study explored and described the delivery of quality nursing care from the perspective of nurses. Data were gathered by tape recorded interviews, published literature and some participant observation. Ten registered nurses from an acute-care public hospital located in Perth, Western Australia were interviewed. Twelve additional transcripts from interviews conducted by postgraduate students were also used. Quality nursing care was perceived to relate to the degree to which patients’ physical, psychosocial, and extra care needs were met. The consequences of quality care were interpreted as ‘therapeutic effectiveness’, where the therapy provided by nurses was perceived to positively affect patients’
  • 63. healing. This was gauged by the patient’s psychosocial and physical response to illness, safety, and satisfaction. Therapeutic effectiveness was facilitated by the development of positive relationships between nurses and patients, nurse’s positive attributes and competent practices, as well as a functional nursing team. The problem of nurses’ inability to consistently provide quality nursing care to all patients was identified. Insufficient time (caused by a lack of human and physical resources) was perceived as the main reason for this. Dissatisfaction and stress in nurses was related to this problem. To deal with this, nurses used a process named ‘selective focusing’. Work was planned to most effectively utilize the time available, within the parameters of safety. Implications for nursing practice, education and management are discussed, and directions for further research are provided. NURSE-STAFFING LEVELS AND THE QUALITY OF CARE IN HOSPITALS Jack Needleman, Ph.D., Peter Buerhaus, Ph.D., R.N., Soeren Mattke, M.D., M.P.H., Maureen Stewart, B.A., and Katya Zelevinsky N Engl J Med 2002; 346:1715-1722May 30, 2002 Background It is uncertain whether lower levels of staffing by nurses at hospitals are associated with an increased risk that patients will have complications or die. Methods We used administrative data from 1997 for 799 hospitals in 11 states (covering 5,075,969 discharges of medical patients and 1,104,659 discharges of surgical patients) to examine the
  • 64. relation between the amount of care provided by nurses at the hospital and patients' outcomes. We conducted regression analyses in which we controlled for patients' risk of adverse outcomes, differences in the nursing care needed for each hospital's patients, and other variables. Results The mean number of hours of nursing care per patient- day was 11.4, of which 7.8 hours were provided by registered nurses, 1.2 hours by licensed practical nurses, and 2.4 hours by nurses' aides. Among medical patients, a higher proportion of hours of care per day provided by registered nurses and a greater absolute number of hours of care per day provided by registered nurses were associated with a shorter length of stay (P=0.01 and P<0.001, respectively) and lower rates of both urinary tract infections (P<0.001 and P=0.003, respectively) and upper gastrointestinal bleeding (P=0.03 and P=0.007, respectively). A higher proportion of hours of care provided by registered nurses was also associated with lower rates of pneumonia (P=0.001), shock or cardiac arrest (P=0.007), and “failure to rescue,” which was defined as death from pneumonia, shock or cardiac arrest, upper gastrointestinal bleeding, sepsis, or deep venous thrombosis (P=0.05). Among surgical patients, a higher proportion of care provided by registered nurses was associated with lower rates of urinary tract infections (P=0.04), and a greater number of hours of care per day provided by registered nurses was associated with lower rates of “failure to rescue” (P=0.008). We found no associations between increased levels of staffing by registered nurses and the rate of in-hospital death or between increased staffing by licensed practical nurses or nurses' aides and the rate of adverse outcomes.

Conclusions A higher proportion of hours of nursing care provided by registered nurses and a greater number of hours of care by registered nurses per day are associated with better care for hospitalized patients SUMMARY Till now we have discussed about the duties and responsibilities of various categories of nursing personnel that includes aims of nursing personnel management, objectives of nursing personnel, various categories of personnel – clinical or institutional level, college level, school level and at community level. CONCLUSION Personnel management means the task of dealing human relationships with in an organization. Aim of nursing personnel management is to employ those nurse best fitted with education, skill and experience to perform the prescribed work.
