Abstract about metabolic balance®
Metabolic balance® is a nutritional program with the goal of supporting participants who wish to make long-term changes in their dietary habits. To accomplish this goal, we only recommend normal foods that can be purchased economically in any food market. No additional products are required, such as nutritional supplements, powders or medications!
Eighty percent of participants are overweight, and many also suffer from disturbances related to the metabolic syndrome, such as type 2 diabetes mellitus, disturbances in lipid metabolism and arterial hypertension.
Our program is based upon the hypothesis that every individual is capable of producing all the enzymes and hormones required for his/her own metabolism. The building blocks necessary for this process must be provided by the diet.
Using personal information and the results of laboratory tests, we design an individual nutritional plan for each participant. The basis of this nutritional plan conforms to the recommendations of the Joslin Diabetes Center and the well-founded nutritional medical concepts of Walter Willett (Professor of Epidemiology and Nutrition, and Chair of the Department of Nutrition at Harvard School of Public Health, and Professor of Medicine, Co-author of the Nurses` Health Study and Health Professionals` Study). Nutritional conversion takes place in 4 phases. During this period, participants are counseled at least 10 times by physicians, nutritional advisors and natural health practitioners who are specially trained in the method. It is important that every participant learn 8 fundamental rules, which should be adhered to life long, if at all possible. A research study conducted by the Hochrhein-Institut für Rehabilitationsforschung, Bad Säckingen (Department for Quality management & Social Medicine at the Freiburg University Medical Center, Director: Prof. Dr. med. Wilfried H. Jäckel) showed that maintenance of good adherence to the program after one year was directly related to adherence to the 8 fundamental rules, and resulted in improvements in important laboratory parameters and in health-related quality of life (IRES-24 score). 64.3% of participants were still able to maintain a weight loss greater than 5% that they had achieved after one year, and 32.6% maintained a weight loss greater than 10%.

The metabolic balance nutritional program does not require taking any pills or powders, and you don’t have to count peas or put a lock on your refrigerator.
The foundation of our method is based upon the hypothesis that a number of different enzymes and hormones are required for the digestion of different foods. Every body is capable of producing these substances internally, just as is described in current textbooks of biochemistry, once it is provided with the necessary building blocks through the diet.
If these enzymes and hormones are restored to balance, then the body is also able to use its own internal signals to decide which foods are still required to maintain this newly balanced metabolism.
Which approach would you recommend to someone who came to you for advice because he/she was overweight? What recommendations do most overweight individuals receive from their physicians and their therapists? The most common recommendations are:

At metabolic balance, we have no fear of fat, and we provide somewhat lower amounts of carbohydrates, but only of the highest quality (45%); we recommend only 3 meals divided through the day, and a diet with the right number of calories.
Excessive carbohydrate consumption rapidly lead to large fat deposits, as is readily seen when we look at Michelangelo’s poor David, who was on loan for two years in the USA. Two years ago, he received a metabolic balance nutritional plan and today, we are happy to report that he once again looks the same as he did before.
Our ancestors who were still wandering about as hunters and gatherers very rarely encountered these short-chained carbohydrates. They lead to a rapid and steep increase in both blood sugar and blood insulin levels. Because of the elevation in the insulin level, the sugar is very rapidly metabolized and within 1½ to 2 hours, a person will experience renewed attacks of hunger, which, of course, are most easily calmed by eating more short-chained carbohydrates.
At metabolic balance, we prefer these carbohydrates with long chains. They lead to a slow rise in blood sugar with a correspondingly slow rise in the insulin level. Over more than 5 hours, we thus obtain a balanced blood sugar level, which leads to lasting satiety.
In studies conducted on mice, scientists at the ETH Zürich have found that insulin levels continuously elevated through constant feeding suppress a protein (FOXA2), which stimulates the urge to be active.

Continuous elevation of the blood insulin level leads to the following changes: elevation of triglycerides and cholesterol, elevation of the stress hormones, adrenalin and cortisol, which in turn leads to elevated blood pressure and increased fat synthesis. In addition, fat synthesis (lipogenesis) is independently stimulated by the activation of lipoprotein lipase (LPL), which has the task of splitting off individual fatty acids from fats, so that these smaller units can penetrate the cell membrane and enter the fat cells. Elevated insulin levels thus lead inevitably to massive increases in fat synthesis. Similarly, individual steps in blood clotting (hemostasis) are activated by the elevated insulin level, which can lead to more frequent thrombosis and embolism in overweight persons. On the other hand, the elevated insulin levels block the formation of very important hormones, such as melatonin, DHEA (dehydroepiandrosterone) and HGH (human growth hormone). These hormones all have powerful anti-inflammatory and anti-oxidant effects and are therefore especially important for persons with inflammatory illnesses. DHEA is a precursor substance for the synthesis of all male and female sex hormones. This is certainly a reason that women with hormone disturbances, such as during the menopause years or during their monthly periods report very significant improvements in their symptoms from the metabolic balance® program. Seventy to eight percent of women who are taking medication during menopause (typically estrogens) are able to reduce the dosage of their medication while following the metabolic balance® program or even eliminate it altogether. This is also true of pre-menstrual symptoms, often characterized by migraines or abdominal pain. Men with low testosterone levels after andropause frequently report that during metabolic balance, they experience a return of their libido.
Whereas lipoprotein lipase is activated by insulin in fat cell membranes and thus leads to increased fat synthesis in fat cells, insulin also blocks lipoprotein lipase in muscle cell membranes, which allows less fat to reach the muscle cells where it could be burned. Elevated insulin levels thus lead to both increased fat synthesis and reduced fat breakdown. These two reactions together with the increased hunger and appetite caused by elevated insulin levels are three points that every client of metabolic balance is required to memorize and remember.
The metabolic balance® nutrition pyramid, based in part on Prof. David Ludwig’s LOGI pyramid, brought to Germany by Dr. Worm, has fresh fruit, vegetables, salads, rye bread and potatoes in the skin on its first level. The oil that is on the 5th level in the DGE pyramid is used for preparation and already included on the first level. On the 2nd level appear animal and plant proteins; grain products that are the foundation for DGE nutrition are here demoted to the second-to-last level. From time to time here, a glass of red wine or beer is also recommended. The positive characteristics of red wine in relation to Type 2 Diabetes and coronary heart disease are generally recognized.
For us as well, sweets are on the very last level, along with soft drinks with their high fructose and sweetener content, which, in very large amounts, promote fat synthesis.
In our system, we work with the revised German Nutrient Database, which lists all available foods along with all of their constituents. In a second container, we have the personal information about our clients, including their body mass, age, gender, previous illnesses, medications they are taking along with their distastes and food intolerances that must be taken into consideration in designing an individual nutritional plan. Should specific minerals be lacking in the blood tests, then foods will be selected that are rich in these constituents; should specific values be high, such as uric acid, then foods will be selected with a low uric acid content. Thus, it often happens that foods that the client has eaten very frequently do not appear in the nutritional plan, since the client already has sufficient amounts of the nutrients contained in these foods. For this reason, foods may also appear on the list that he/she has eaten less often recently, or even foods he/she has never eaten before at all.
