Each dimension section has ten statements that define an aspect of whole-person
wellbeing. You’ll rate each statement with the slider bar, in two different ways:
• How True is it for you right now? 1= not true for me now, 10 means completely true.
• How Motivated are you to improve? 1= Not motivated now, 10 = very motivated .

Rate your daily or weekly

Action Steps.
Tracking allows you to see
how you’re doing and
reminds you where to focus.
When you have integrated a

Successes. This makes
room to create another step!
