ICD-10 Documentation Strategy
1. Find the diagnostic statement from a recent patient note.
2. Locate the ICD-9 codes reported for that service.
3. Search for the ICD-10 code using one of these three methods:
a. Code mapping (i.e. GEMs tables)
b. A common ICD-10 code list
c. Alphabetic Index
4. Look up the code in the Tabular List and review the 4th, 5th,
6th, and/or 7th character.

5. Review in-column instructions at the level of each
character. Pay particular attention to inclusion, Excludes1,
and Excludes2 notes.
6. Compare the new codes to the existing diagnostic
statement.
7. Re-create the note to ensure that it supports the newly
selected ICD-10 codes.
8. The next time a patient presents with that condition,
document in the new way.

Look up the code in the Tabular List and review the 4th, 5th, 6th, and/or 7th character. 6
