4. WHAT are we? Citizens Care and Rehabilitation Center (Citizens): 170 bed Skilled Nursing Facility and Rehabilitation Center located on Rosemont Ave, across from Fort Detrick Resident skilled nursing & custodial care paid for by Medicare, Medicaid, private insurances, and private pay funds Approximately 220 staff members provide services, and include: Nurses, Aides, ancillary, and administrative staff. Also employs a Medical Director (physician), rehabilitation therapists, psychologists and other professionals. Received a four out of five Star rating by Centers for Medicare & Medicaid Services for the past two years
5. WHAT are we? Montevue Assisted Living (Montevue): All private room Assisted Living Facility located adjacent to Citizens, with an entranceway off of Montevue Lane Resident care paid for with private pay funds and County subsidies when necessary Residents can participate in Medicare B, Medicaid, and other private insurance programs Approximately 40 staff members care for our 60 residents, along with a Medical Director, rehabilitation staff, and other professionals.
6. How are we the SAME? Both facilities are owned and operated by Frederick County Government, governed by a volunteer Board of Trustees and the County Commissioners Both are licensed by the Maryland Department of Health & Mental Hygiene and receive annual quality surveys Both provide family-oriented care with the best interest of the residents in mind. Both provide end of life care when needed
7. How are we DIFFERENT? Citizens: Skilled Nursing Facility, or what used to be called a âNursing Home.â Must meet specific clinical criteria in order to receive insurance coverage for an admission. Most commonly receive admissions from hospitals, other nursing homes, or assisted living facilities. Montevue: Assisted Living Facility, allowing people who may not be able to function ideally in a home setting any longer to live in a home-like atmosphere with some assistance from professional and ancillary staff members. Residents can perform many of their Activities of Daily Living (ADLâs) independently, but may need varying levels of assistance.

Some History Citizens: Built in 1976 to house 120 residents , âDâ Wing completed in 1981, adding 50 beds, a dining room, activities room, and indoor gardens to meet expanded need and add home-like touches. Montevue: built in 1870 as a solution, at the time, to help care for Frederick Countyâs frail elderly. Moved across Montevue Lane in 1987 to newly constructed facility with 60 resident rooms, all private.

11. Who are our Customers? Residents & families of Frederick County and beyond who need skilled nursing, rehabilitation, or assisted living services and meet specific criteria Those needing short term physical, speech or occupational rehabilitation prior to a return home Those needing long term care who are dependent (Citizens) or somewhat independent (Montevue) in their activities of daily living Those who suffer from dementia and other chronic health conditions Those who are in the end stages of life

13. What Services Do We Offer? Montevue: Assisted Living Services - activities, meals, laundry, assistance with Activities of Daily Living Citizens: Respite Care as well as Palliative and/or Hospice Care Restorative. Rehabilitative, and IV Antibiotic Therapies Specialized services such as: Dementia Care Therapeutic Recreation Specialized Diet Management & Care 24 Hour Professional Nursing Wonderful quality of life for those who need safety, security, and social services and can no longer live independently or be cared for at home
15. Where and Why New Facilities? Brand-new Assisted Living and Skilled Nursing facilities on the same site - just across the parking lot from our current location, on Rosemont Avenue across from Fort Detrick For Montevue, all residents will live in graciously appointed private efficiencies, reaturing private bathrooms and âkitchenettesâ For Citizens, gone will be triple person âwards.â Instead, all will enjoy increased privacy in either private or semi-private rooms with full walls separating living spaces (residents in doubles will still share a bathroom). We will be able to better meet the needs of our residents with state of the art design & programming

17. When Do We Move In? The new facilities will open at the end of 2011 or beginning of 2012, with open house tours beginning sometime before we move our residents into their new home

19. What are the New FacilitiesLike? Citizens will feature: 74 attractive Private Rooms, 48 Semi Private rooms with private spaces for beds and private bathrooms - all with furniture, electric beds, call bell system and curtains Several comfortably large recreation spaces/rooms for the many activities, as well as areas for residents and families to meet publicly and privately Graciously sized and appointed dining rooms on every resident neighborhood, featuring seated service & individual menus Catered dining timed around when each resident wishes to eat Beauty Salon, Dental & Podiatry Suite, and Gift Shop Special lifts for bariatric residents Oxygen piped into the rooms â no more bulky tanks or unsightly equipment required!

(continued) Relaxing whirlpools for bathing residents Maximum safety and security, provided via: Life safety system with smoke detectors, sprinklers, and ennunciators to provide maximum safety and security Added security of Video Surveillance Cameras and monitors of inside and outside of the building Beautifully furnished lobbies and public waiting areas And the same dedicated, and deeply caring staff who have provided loving care to the residents of Citizens since 1976

What New Services Will Be Added? 16 bed Ventilator unit managed by professional Respiratory Therapists who are trained to care for residents needing tracheotomy, ventilator, and COPD services Increased short term physical, speech, and occupational therapy services Outpatient Rehabilitation Services for the general public Specially designed and programmed Dementia Care unit with staff trained specifically for dementia care: Programming designed to help residents maintain their dignity and as much independence as possible for as long as possible A unique space called a âSnoozelin Roomâ designed to provide residents with gentle relaxation via soothing music in a calming, attractive atmosphere Separate, secure unit to keep wanderers safe
