CCK
CCKImmediately following the webinar, you can review the presentation and listen to it again. All of the questions asked during the presentation will be posted in the FAQ document on June 8.
CCKPlease have RFA guidance documents available either printed or on your screen so that you can refer to them throughout the webinar.
CCKThis morning, we’ll go over the various components of the RFA and periodically break for questions you might have.
CCK
CCKIntent of the CCPD RFA is to select applicants to receive grant funding for prevention, early detection, and treatment of CCPD through a competitive review process.
CCK
CCKThere is not a suggested maximum award amountNumber and amount of grant awards will vary depending on the number of awarded applications, geographic area and population reach, proposed activities, community needs, and recommendations made during the grant review process. CCPD Review Committee reserves the right to negotiate budgets and work plan with selected eligible applicants depending on the number of eligible applicants.A minimum of 10% of available funds are reserved for rural areas except if CCPD RC determines that there are no adequate proposals targeted toward rural areasIn addition, a minimum of 10% of available funds shall be directed to each of the following 3 disease areas: cancer, CVD, and PD, expect if the CCPD RC determines that there are no adequate proposals in a given disease area. Funding levels for years 2 and 3 may be up to the annualized (12 months vs. 9 months) amount requested for the current fiscal year or year 1.
CCKThe lead agency will assume responsibility for overall program implementation, fiscal oversights, and reporting. Partnerships must be clearly identified and letters of collaboration from participating agencies must state the partnership or support offered.

CCK
CCKExample risk factors include overweight, high cholesterol, hypertension, and high glucose levels or impaired glucose intolerance
CCKSurveillance and epidemiology (and evaluation) work will be separate from the competitive grants program and will be part of an interagency agreement with an evaluation vendor that will conduction surveillance and evaluation activities.
CCK
CCKThis criteria refers to statutory requirements of the grant program as a whole and we wouldn’t expect one application to address all criteria. For example, in the first bullet, an applicants is not expected to translate evidence-based strategies in all three settings. Addressing one setting is sufficient.
CCKYou will be asked to select which category you are applying for in the grants management systemYou will be asked to select which goals (cancer, cardiovascular disease, or pulmonary disease) in the grants management system
CCKIn the work plan, you will be asked to select which public health domains your project’s objectives will meet.
CCKWithin this category, applicants can propose activities within a region of Colorado—it is not limited to one county or city. Examples of applicants for this approach: LPHAs, community-based organizations, local nonprofits, Federally Qualified Health Centers, federally recognized Native American Tribes, community health centers, county nursing services, education agencies, local aging centers, senior centers, and universities.See the application guidelines for example strategies for community level approaches—these strategies are examples of what an applicant organization can propose. An applicant will not lose points if the organization proposes activities for one strategy—there is no minimum number of strategies an applicants has to apply for.

CCK
CCKMany of the questions are BOH required questionsThe character limit for each question are available in COGMS online application instructions. Since the applications are electronic, there are no page limits like past RFAs. We recommend answering the questions in Word and checking the character amounts before entering answers into COGMS. Letters of support/collaboration only required if:Submitting a multi-agency application: Partnerships must be clearly identified and letters of collaboration from participating agencies must state the partnership or support offeredIf applying for a community-level approach, but are not a LPHA. Letters of collaboration/support from LPHAs located within the targeted geographic area specified in proposal are required. Specific section in COGMS to identify collaborating/supporting agencies and upload letters. Refer to online application instructions for guidance on how to enter this information.
CCKA template for the work plan is not provided, but COGMS will walk you through the steps needed to develop a work plan. When entering each Specific, Measurable, Achievable, Realistic, and Time-Framed objective into the work plan, select one or more public health domains that the objective supports. If multiple strategy types will be employed in reaching the objective, you will need to repeat the objective for the corresponding number of strategy typesOutcome: enter intended results or accomplishments of implemented activitiesPopulation: enter specific individuals you intend to impact with this objective—this may be a subpopulation of your project’s target population or a specific group of individualsList up to 10 activities that will occur to support the objectives and provide specific information about what will be done where and how oftenMeasurement: Reach is the number of individuals known to have received an intervention designed to improve their health an/or lower their disease riskImplementation includes details that will explain how, where, what, when, and who of your project’s activities.Effectiveness includes measures or data that demonstrates that the proposed objective was effectively implemented to achieve the desired outcome
CCK –Because these questions ask for a summary of detailed information provided in the work plan, it is advisable to complete the work plan prior to developing responses to evaluation questions.

CCKApplications will be scored through a competitive review process.
CCK
CCKIn addition to the evaluation criteria listed in the RFA, the CCPD review committee adopted these principle to consider when prioritizing applicants:Data driven: interventions/programs that address those conditions that contribute most to the current burden of disease in ColoradoEvidence-based: high level of evidence for effectiveness of interventions/programs as determined by the scientific literatureEvaluation: indicators available or can be developed to measure impactGreatest reach/impact: interventions/programs that have the greatest impactCoordination: interventions/programs that promote prevention, early detection, and treatment to comprehensively address chronic disease at statewide and community levelsRisk factor reduction: strategies that aim to decrease interrelated chronic disease risk factors
