Performance Appraisal Flowchart
Performance Appraisal Meeting Scheduled
The manager confirms and sends the appropriate paperwork to the appraisee allowing a
minimum of 5 working days notice.
Appraisee and appraiser complete their preparation forms.
These documents will aid the discussion but do not have to be submitted as part of the
process.
Performance Appraisal Meeting is held
Appraisal is a two way dialogue, not a form-filling exercise. The focus should be the
conversation. Notes should be jotted down but the paperwork should be completed after the
meeting
Appraisal documentation completed and signed
The manager completes the final performance appraisal form and gives 2 copies to the
appraisee for signing. The signed copies are then returned to the Operational Manager
Operational Manager collates all appraisal documentation for the Group/Service
A Group Learning and Development Form is completed and a meeting arranged with the
Assistant Director to sign off all appraisals and discuss learning and development for the
group. The Operational Manager provides a fully signed copy of the appraisal form to the
appraisees.
Assistant Director signs off appraisals for all his Groups/Services
A Directorate Learning and Development Form is completed and sent to Human Resources
Assistant Director sends all appraisal documentation to Human Resources
Human Resources scan and file all appraisal forms on personal files. The Directorate Learning
and Development Form is used to formulate the Corporate Training Programme for the
following year.
